慢性肾功能不全时测定左心室收缩时间间期的意义

摘  要:本文报道了36例慢性肾功能不全患者的左心室收缩时间间期测定结果,并探讨其临床意义。作者认为,该项检查在慢性肾功能不全患者中有实际应用的价值;有关PEP/LVET比值和多种病理因素的相关分析表明,尿毒症晚期心肌病变确非单一因素引起。A study of systolic time intervals(STI) in 36 patients with chro- nic renal insufficiency (CRI) was reported.36 cases of CRI (male 19, female 17) ranging in age from 21 to 65-years (mean 34.5 years) were divided into three groups: group A included 15 cases of late stage uremia, group B was composed of 10 cases of early stage uremia, and group Cincluded 11 cases of uncompensatory stage of renal insufficiency.The four basic STI are pre-ejeetion Period (PEP), left ventricular ejection time(LVET), total electro-mechanical systole(OS2), and PE P/LVET ratio. STI were measured from the simultaneous recordings of ECG, CPT, CPG, ACG by an Mingograf recording system 34 type. In addition, the other noninvasive methods were applied to these patients, such as UCG, chest X-ray examination Laboratory data (Hb, cholesterol, anion gap, Cr) were also determined in every cases.The mean values of PEP/LVET in group A, B, and C were 0.57 0.45) and 0.41 respectively. There was significant difference in three groups(P<0.01). The increase of mean values of PEP/LVET was with the damage degree of renal function. The results suggested that STI are highly useful to establish the role of left ventricular muscle dysfunction in patients with renal insufficiency. The ratio PEP/LVET is the most sensitive indicator of left ventricular dysfunction.An analyse of eorrelation between the values PEP/LVET and some factors such as blood pressure, Hb, cholesterol, anion gap, Cr had also been made in these patients. The result suggested the uremic myocardiopathy was not r(?)ally caused by any single factor.
