摘 要:目的:通过将藻酸钙凝胶、自体微小颗粒骨复合构建可塑形组织工程骨,探索一种新型、复合型骨缺损修复材料,为临床骨缺损的修复提供一种新的方法及理论依据。方法:随机将36只日本大耳白兔分成4组,并于两侧桡骨干处造成1.5cm缺损。A组,植入以藻酸钙凝胶为载体的自体微小颗粒骨;B组,植入自体微小颗粒骨;C组,植入藻酸钙凝胶;D组,不植入任何物质的空白组。术后4周和8周分别行大体观察,X线摄片,组织学观察及术后8周行生物力学测定。结果:4周和8周通过大体,X线片以及组织学观察的结果显示A,B,C组骨缺损区均得到了的修复,但A组在成骨速度,骨再生量,再生髓腔结构等方面均优于B,C组。8周的生物力学实验显示:3点弯曲试验结果表明A组强度极限值明显高于B,C组,差异经统计学分析有显著性意义(P<0.05)。D组则不能产生骨性愈合。结论:藻酸钙凝胶可作为微小颗粒骨的载体。以藻酸钙凝胶为载体的自体微小颗粒骨成骨量多,质量好,骨的机械强度高,修复骨缺损的能力较单纯自体微小颗粒骨和单纯藻酸钙凝胶强。Objective:To explore a new and compound type of material through tissue engineered bone prefabricated with combination of calcium alginate gelatin and autogenous micromorselized bone,in order to provide a new method and theoretical support about the repair of clinical bone defect.Methods:36 Japan white big ear rabbits were divided into 4 groups randomly,then bone defects were created on the bilateral radius.A group was implanted the autogenous micromorselized bone of the calcium alginate gelatin as the carrier;B group was implanted the autogenous micromorselized bone;C group was implanted the calcium alginate gelatin;D group was implanted nothing.The bone defects were observed by gross examination,X-ray examination,histological examination at 4w,8w and it were determined by biomechanics at 8 w.Results:The results of gross examination,X-ray examination,histological examination showed that defects treated with A,B and C implantation were repaired at 4w and 8w,but the defects treated with A implantation regenerated much more new bone,bridged earlier and were repaired more quickly than that of B and C.The results of biomechanical examination were as below at 8 w:three points bent test showed that A group was better than B group and C group according to the anti-bending degree and max stress.The results had the significant statistic difference(P0.05).While D group couldn't get osseous tissue healing.Conclusion:The calcium alginate gelatin may be the better carrier of the autogenous micromorselized bone.The autogenous micromorselized bone of the calcium alginate gelatin as the carrier formed more bone whose quality and biomechanics is excellent,and has better capability on repairing the bone defect than the autogenous micromorselized bone or the calcium alginate gelatin along.
