宫颈阴道分泌物IL-8和MMP-8与早产的关系

摘　　要：目的:探讨宫颈阴道分泌物中IL-8及MMP-8与早产的关系。方法:收集42例妊娠28～36+6周诊断为先兆早产的孕妇(研究组)和40例同期且孕龄相仿的正常孕妇(对照组)的宫颈阴道分泌物,采用ELISA法测定IL-8及MMP-8含量。结果:①研究组宫颈阴道分泌物IL-8及MMP-8水平均明显高于对照组,P<0.05。②以90pg/ml为最佳临界点,宫颈阴道分泌物IL-8预测早产的敏感度、特异度、阳性预测值、阴性预测值分别为88.5%、84.3%、73.7%、91.5%;以20ng/ml为最佳临界点,MMP-8预测早产的敏感度为76.9%,特异度为66.7%,阳性预测值为50.0%,阴性预测值为83.8%。③IL-8及MMP-8阳性组与阴性组相比,均显示阳性组的分娩孕周更小,取样至分娩的间隔天数更少,新生儿体重更低,均P<0.05。④宫颈阴道分泌物中IL-8和MMP-8呈显著正相关,相关系数r=0.671,P<0.05。结论:①宫颈阴道分泌物中的IL-8和MMP-8与早产有相关关系。②IL-8和MMP-8呈显著正相关。③IL-8和MMP-8阳性与不良妊娠结局有关。Objective： To explore the relationship between intedukin-8 （IL-8）, matrix metalloproteinase -8 （MMP-8） and preterm labor in cervieovaginal secretions. Methods： The cervicovaginal secretions from 42 pregnant women of 28 - 36^ ＋6 gestational weeks with threatened preterm labor （study group） and 40 normal pregnant women of the same gestational weeks （control group） were collected, the levels of MMP - 8 and IL - 8 in cervicovaginal secretions were measured by ELISA. Results ： The levels of MMP - 8 and IL - 8 in cervicovaginal secretions in study group were significantly higher than those in control group （ P 〈 0. 05 ） ; selecting 90 pg/ml as critical point, the sensitivity, specificity, positive predictive value and negative predictive value of IL - 8 in cervicovaginal secretions in predicting preterm delivery were 88.5% , 84. 3% , 73.7% and 91.5% , respectively; selecting 20 ng/ml as critical point, the sensitivity, specificity, positive predictive value and negative predictive value of MMP - 8 in cervicovaginal secretions in predicting preterm delivery were 76. 9% , 66. 7% , 50. 0% and 83.8% , respectively ; compared to negative expression group of IL - 8 and MMP - 8, positive expression group of IL - 8 and MMP - 8 had smaller delivery gestational weeks, shorter interval time between sampling and delivery, lower neonatal weight （ P 〈 0. 05 ） ; there was a positive correlation between IL - 8 and MMP - 8 in cervicovaginal secretions （ y = 0. 671, P 〈 0.05 ） . Conclusion ： There is correlation between IL- 8, MMP- 8 and preterm labor; there is a positive correlation between IL- 8 and MMP- 8 in cervicovaginal secretions ; positive expressions of IL - 8 and MMP - 8 are related to adverse pregnancy outcomes.
