摘 要:目的 探讨31磷(31P)磁共振波谱(MRS)用于实验及临床评价肝病的价值.方法 分别对30只家兔和30名健康志愿者肝脏在2个不同时间段内行2次MRS二维化学位移成像(2D CSI),通过检测6种磷代谢物的相对值,分析家兔和人体肝磷代谢物的检测误差及影响因素.结果 (1)在视野为280mm×280mm、采集次数为40次、呼吸门控下采集,人肝磷代谢物变异系数分别为:磷酸单脂(PME)34.53%、磷酸二脂(PDE)21.53%、无机磷(Pi)37.41%、α-三磷酸腺苷(ATP)16.96%、β-ATP 13.34%、γ-ATP 17.78%;各磷代谢物的变异系数在12.44%~37.41%之间,其中以β-ATP重复性最好,误差在13%左右;2次检测各磷代谢物结果 的差异均无统计学意义(P〉0.05).(2)在视野为200mm×200mm、采集次数为40次、无呼吸门控时采集,兔肝磷代谢物变异系数分别为:PME 36.21%、PDE 21.25%、Pi 33.73%、α-ATP 11.65%、β-ATP 9.25%、γ-ATP 18.41%;各磷代谢物的变异系数在8.48%~36.21%之间,其中以β-ATP重复性最好,误差均在10%以内;2次检测各磷代谢物结果 的差异均无统计学意义(P〉0.05).结论 ATP相对值可能是评价实验及临床肝病共同最可靠的指标,而家兔与人体肝脏MRS成像特点及影响因素却不尽相同.Objective To evaluate liver function by ^31P magnetic resonance spectroscopy (MRS) with two dimensional chemical shift imaging (2D CSI) in rabbits and human subjects. Methods Liver scan with 31P MRS was performed in 30 rabbits and 30 health human volunteers twice in a time interval, and the reproducibility of scan in 2 groups was analyzed with t-test. The relative quantification of phosphorus metabolites of liver was detected with 2D CSI by 1.5T MR scans, and the influencing factors of 2D CSI technique were analyzed. Results Under the condition of FOV 200 mm, average 40 times and without respiration gating, the coefficient of variation (CV) of metabolites within 30 rabbits was 36.21% for phosphomonoesters (PME), 21.25% for phosphodiesters { PDE), 33.73% for inorganic phosphate { Pi ), 11.65% for adenosine triphosphate (a -ATP), 9.25% for 15 -ATP and 18.41% for y -ATP, respectively. Among the metabolites, 13 -ATP was most reproducible with the CV than 10%. No significant changes of metabolites were detected in inter-assay (P〉 0.05). With FOV 280 mm, average 40 times and under respiration gating, the CV of metabolites in 30 healthy volunteers was 34.53% for PME, 21.53% for PDE, 37.41% for Pi, 16.96% for a -ATP, 13.34% for I5 -ATP and 17.78% for ^-ATP, respectively. Among the metabolites, 15 -ATP was also the most reproducible. No significant changes of metabolites were detected in inter-assay (P 〉 0.05). Conclusion Hepatic ATP levels may be the most reliable criteria for experimental or clinical liver diseases, but for rabbits or human subjects the characteristics and influencing factors of liver 31P MRS may be of some differences
