摘 要:目的探讨孕前体质指数(BMI)、孕期体重增加值与新生儿体重、妊娠期高血压疾病的关系。方法收集并分析395例足月初产孕妇身高、孕前体重、孕期体重增加、新生儿体重、妊娠期高血压疾病的发病情况的数据,评价孕前体质指数、孕期体重增加值与新生儿体重、妊娠期高血压疾病的相关性。按孕前体重指数将观察对象分为低体重组(BMI〈18.5 kg/m2),正常体重组(BMI18.5-24.9 kg/m2),超体重组(BMI≥25.0 kg/m2);按孕期体重增加值〈11.0kg、11.0-17.9 kg、18.0-24.9 kg、≥25.0 kg将观察对象分为四组;按出生体重〈2500 g、2500-3999 g、≥4000 g将观察对象分为低体重出生组、正常体重出生组、巨大儿组;按产前血压140/90 mm Hg(1 mm Hg=0.133 kPa)水平将观察对象分为正常组及高血压组。结果①不同孕前体重指数组间新生儿出生体重比较差异无统计学意义(P〉0.05)。②不同孕期体重增加组间新生儿出生体重比较差异有统计学意义(P〈0.05),随孕期体重增加值增大,新生儿出生体重相应增加。③不同孕前体重指数组间妊娠期高血压疾病发生率比较差异有统计学意义(P〈0.05),不同孕期体重增加组间妊娠期高血压疾病发生率比较差异有统计学意义(P〈0.05),随孕前体重指数增加及孕期体重增加值增大妊娠期高血压疾病的发生率增加。结论合理调整孕前体重指数及孕期增加体重,可以在保持新生儿体重正常的情况下减少妊娠期高血压疾病的发生率。Objective To investigate the association between pre-pregnancy body mass index(pBMI),pregnancy weight gain and newborn's weight,incidence of hypertensive disorders in pregnancy.Methods We collected and analyzed data from the height,pre-pregnancy weight,pregnancy weight gain of 395 full-term primiparae,newborns' weight and the incidence of hypertensive disorders in pregnancy,evaluated the relationships between pre-pregnancy body mass index(pBMI),pregnancy weight gain value,and newborns' weight,incidence of hypertensive disorders in pregnancy.According to pBMI,the women were divided into underweight group(BMI18.5 kg/m2),normal weight group(BMI 18.5-24.9 kg/m2) and overweight group(BMI≥25.0 kg/m2).According to pregnancy weight gain value(11.0 kg,11.0-17.9 kg,18.0-24.9 kg,≥25.0 kg),the women were divided into 4 groups.According to birth weight(2500 g,2500-3999 g,≥4000 g),the women were divided into underweight birth group,normal weight birth group and macrosomia group.According to the level of prenatal blood pressure,the women were divided into normal group(140/90 mm Hg) and hypertension group(≥140/90 mm Hg).Results ①No statistically significant difference across pBMI groups in birth weight(P0.05).②Birth weight showed statistically significant difference across pregnancy weight gain groups(P0.05),and with the value of weight gain during pregnancy increases birth weight showed a corresponding increase.③The incidence of hypertensive disorders in pregnancy showed statistically significant difference across pBMI groups(P0.05).The incidence of hypertensive disorders in pregnancy showed statistically significant difference across pregnancy weight gain groups(P0.05).With the increases of pBMI and the value of pregnancy weight gain the incidence of hypertensive disorders in pregnancy showed a corresponding increase.Conclusion The rationalization of pre-pregnancy body mass index(pBMI) and pregnancy weight gain can reduce the incidence of hypertensive
