摘  要:本项研究通过对日本血吸虫再感染病例及新感染病例多项免疫学指标进行了比较.结果显示反复感染病人对再次感染的炸波免疫应答,明显强于初次感染病例,细胞免疫系统对再次感染的应答有所减弱,提示人类对日本血吸虫获得性免疫力主要是通过特异性抗体应答或其协同作用来实现的。对两组病例血清同型、亚型抗体水平检测显示,新感染组特异性IgM显著高于再感染组(P<0.05-0.01),后者除IgM外,其它各型均高于新感染组,而以IgE为最显著。由日本血吸虫卵抗原(SEA)诱导T细胞应答结果表明,2组病例T细胞经抗原刺激后,INFγ的生成未见明显增加,但再感染病人的T细胞经SEA刺激后,IL-4的生成有增加的趋势,提示随着日本血吸虫病程的发展,可能形成以Th_2为主的免疫应答。本次研究对新感染和再感染病例血清诱导小鼠巨噬细胞及嗜酸性细胞体外杀童虫的作用显示,感染病人血清能诱导出较强的体外杀童虫作用,这一结果与国内有关报告相一致,进一步证明了ADCC机制是构成日本血吸虫感染获得性免疫力的重要组成部分.Several criteria on humoral and cellular immune responses were compared between primary and reinfected cases of Schistosoma japonica. The overall results showed that the hu-moral responses of the reinfection group were remarkably stronger than those of the primarygroup, while the cellular responses appeared t0 be weaker in the former. This may indicatethat the acquired immunity of human being against S. japonicum was mainly attained throughspecific antibodies or with its cordinated actions. Serum antibody isotype and subclass detections revealed a significantly higher level of specific IgM in primary cases (P<0. 05-0. 01),while in reinfcetion group, antibody types other than IgM were all showed higher levels thanthe primary group, with a particular case in the specific IgE level. In the experiment of SEAinduced T-cell responses, the level of INF-γwas not increased in both groups. But the levelof IL-4 in the reinfection group demonstrated an increased trend after SEA stimulation, suggesting that the Th_2 response might gradually be formed along with the course of the disease.In schistosomula-killing test with macrophages and eosinophils of mice the sera from reinfected cases could induce stronger killing effect, thus indicated that ADCC also constitutedthe part of acquired immunity in Schistosoma japonicum infections.
