心理状态及应激对产妇选择不同分娩方式的影响

摘　　要：目的研究心理状态及应激对产妇选择分娩方式的影响。方法住院分娩产妇共158例,其中要求阴道试产76例,要求剖宫产82例,入院时填写成人艾森克人格问卷(eysenck personality questionnaire,EPQ),焦虑自伴量表(self-rating anxiety scale,SAS),测定外周血儿茶酚胺类激素含量以研究其应激状态。结果试产组中有剖宫产指征产妇SAS阳性率、肾上腺素和去甲肾上腺素含量高于无剖宫产指征者(P<0.05);剖宫产组中无剖宫产指征产妇,EPQ分析中N量表平均T值高于其他3组(P<0.05),其SAS阳性率、肾上腺素及去甲肾上腺素值高于有指征产妇(P<0.05)。结论心理状态及应激影响产妇选择分娩方式,在注重对躯体护理的同时应有针对性的加强心理护理,使产妇选择分娩方式更加正确合理。Objective To study the effects of psychological state and stress on the delivery woman choosing different delivery manners. Methods One hunred and fifty eight delivery women were randomly collected, including 76 cases demanding vaginal delivery and 82 cases demanding caesarean delivery. All of them filled in eysenck personality questionnaire（EPQ） and self-rating anxiety scale（SAS）. The catecholamines in their peripheral blood were measured. Results In the vaginal delivery group, the positive rate of SAS and the level of catecholamine hormone in the delivery women with caesarean delivery indication were significantly higher than those in the delivery women without caesarean delivery indication （ P 〈 0.05） ; In the caesarean delivery group, T mean value in the N column in EPQ table was significantly higher in the delivery women without caesarean delivery indication than in other three groups（ P 〈0.05）. The positive rate of SAS and the level of adrenalin and noradrenalin in the delivery women without caesarean delivery indication were higher than those with caesarean delivery indication（ P 〈0.05）. Conclusion The psychological status and the stress could influence the delivery women to choose their delivery manners. Laying emphasis on the body nursing and simultaneously giving the intensive care in psychology may help the women make the right and reasonable choices for their delivery manners.
