ELISA抗原抗体联合检测生殖器疱疹病毒的实验室应用

摘　　要：目的:探讨早期快速诊断生殖器疱疹(GH)的实验室方法。方法:500例临床上已确诊为由单纯疱疹病毒2型(HSV-Ⅱ)引起的GH患者为实验组,取分泌物和血液分别进行细胞培养、HSV-Ⅱ抗原以及IgM、IgG分类抗体检测;以200例从未感染过HSV-Ⅱ的门诊患者为对照组作同步实验。结果:GH细胞培养法和抗原、抗体检测法特异性分别为100.0%和98.0%、97.5%,后两种方法分别与细胞培养法比较,均无统计学差异(均P>0.10);灵敏度分别为71.8%和88.0%、84.0%,后两种方法分别与细胞培养法比较,均有统计学差异(均P<0.005)。水疱(脓疱)型GH患者3种方法灵敏度分别为77.6%和98.5%、76.4%,糜烂结痂型和不典型GH患者3种方法的灵敏度分别为60.6%和67.6%、98.8%。结论:对水疱(脓疱)型GH患者采用ELISA-HSV-Ⅱ抗原检测法比较理想,对糜烂结痂型和不典型GH患者同时采用ELISA-HSV-Ⅱ-IgM、IgG分类抗体检测法比较理想。Objective： To explore an effective laboratory method for early rapid diagnosis of genital herpes （GH）. Methods： 500 cases of outpatients with GH who had been confirmed being infected with herpes simplex virus type 2 （HSV-U） in clinic and 200 outpatients who had never been infected with HSV-Ⅱ were enrolled into the study. Secretion was collected for HSV-Ⅱ antigen detection and cell culture, Serum was collected for HSV-Ⅱ IgM and IgG. Results： Specificities of cell culture, antigen and antibody detection were 100.0% ,98.0%, 97.5%, respectively.Comparing the antigen and antibody detection methods to the cell culture, respectively, no statistics significant differences was found（ all P 〉0.10） ;Sensitivities were 71.8%, 88.0%, 84.0%, respectively, with statistically significant differences among those using the antigen and antibody detection methods and cell culture method（ all P 〈0.005 ）. In the blisters （impetigo）-type group ,the sensitivities were 77.6% ,98.5% and 76.4%, respectively; in the erosion scar-type and atypical GH group, the sensitivities were 60.6%, 67.6%, 98.8%, respectively. Conclusion： It is an ideal way to detect ELISA-HSV-Ⅱ antigen in blisters （pustular） patients, and to detect HSV-Ⅱ-IgM and IgG in erosion scar-type and atypical GH by ELISA.
