TGF—β1,VEGF抗体等与2型糖尿病肾病患者并发高血压肾间质纤维化指标变化的相关性研究  被引量:7

摘  要:目的探讨转化生长因子(TGF-β1)、血管内皮生长因子(VEGF)抗体、肿瘤坏死因子(TNF—α)、透明质酸(HA)、纤维连接蛋白(FN)、层黏连蛋白(LN)、Ⅳ型胶原(CⅣ)、Ⅲ型前胶原(PCⅢ)在2型糖尿病肾病患者并发高血压肾间质纤维化的发病机理及病程进展中表达的相关性。方法选择经诊断为2型糖尿病肾病并发高血压患者146例;同时选择40例健康正常人作对照组。应用酶联免疫分析法(ELISA)对TGF—β1,VEGF抗体,TNF—α进行检测,用化学发光分析法对HA,FN,LN,CⅣ,PCⅢ进行检测。结果2型糖尿病肾病并发高血压肾间质纤维化患者的TGF—β1,VEGF,VEGF抗体,TNF—α,FN,LN,CⅣ,PCⅢ表达显著上调(P〈0.01),较正常对照组差异有统计学显著性意义(P〈0.01),且与病情的进展呈正相关;而HA表达明显下调,较正常对照组差异有统计学显著性意义(P〈0.01),且与病情的进程呈负相关。结论生长因子(PEDF)是有效的血管生成抑制剂,对保持正常组织透明无血管的状态有重要作用,TGF-β1是促进纤维化的重要细胞因子,调节肌成纤维细胞分化和胶原合成,在肾纤维化发生发展中起着重要的作用。而VEGF是血管和淋巴内皮细胞的生长和分化及正常和异常血管新生的重要的调节因子,在生理和病理性血管新生中起重要作用。三者同时表达异常,可能提示肾间质纤维化患者免疫病理机理中的细胞活化模式,对肾间质纤维化患者的免疫机制和指导治疗有指导意义。Objective To investigate the correlation of serum transforming growth factor-β1 (TGF-β1),vascular endothelial growth factor antibody (VEGF), tumor necrosis factor-alpha (TNF-α), hyaluronic acid (HA), fibronectin (FN), laminin (LN) ,lollagen Ⅳ (C Ⅳ ) and pro-collagen Ⅲ (PC Ⅲ ) expressions in the pathogenesis and progression of renal interstitial fibrosis in type 2 diabetic nephropathy patients with high blood pressure. Methods To select 146 patients diagnosed as type 2 diabetic nephropathy with high blood pressure and 40 healthy persons as control group. TGF-β1, VEGF antibody and TNF-α were detected by enzyme-linked immunosorbent assay (ELISA). HA,FN, LN, CⅣ and PCⅢ were detected by Chemiluminescence Analysis(CfA). Results The expressions of serum TGF-α,VEGF,VEGF antibody,TNF-α,FN,LN, CⅣ and PCⅢ in type 2 diabetic nephropathy patients with high blood pressure and renal interstitial fibrosis were obviously up-regulated (P〈0. 01) and there were significant difference in comparison with normal control group(P〈0. 01). The expressions above were positively correlated with the disease progression. At the same time ,the expression of HA was obviously down-regulated (P〈0. 01) and the difference was significant (P〈0. 01). The expression of HA was negatively correlated with the disease progression. Conclusion Pigment eplthelium-derived factor(PEDF) is an effective angiogenesis inhibitor and it plays an important role in keeping the normal tissues transparent and no blood vessels. TGF-β1 is an important cytokine for promoting fibrosis. It could regulate the myofibroblasts differentiation and collagen synthesis and it is very important for the development of the renal fibrosis. VEGF is an important regulator in the growth and differentiation of the vascular endothelial cells and the normal and abnormal angiogenesis.
