摘 要:目的评估BACTEC Plus/F瓶在人工关节假体感染的患者关节液和组织中细菌培养的优越性。方法58例临床诊断为人工关节感染的患者,手术中抽取关节液和切取假体周围软组织,进行细菌常规培养和BACTEC Plus/F瓶增殖培养。手术中快速冰冻切片病理检查。结果58例病理检查均证实有炎症反应,支持临床诊断。组织匀浆检出率:BACTEC Plus/F培养为79.3%,常规培养为50%,两者差异有统计学意义(P=0.005)。关节液细菌检出率:BACTEC Plus/F为74.1%,常规培养为45.1%,两者差异有统计学意义(P=0.003)。污染率两者差异亦有统计学意义(P=0.041)。结论BACTEC Plus/F瓶人工关节假体感染的患者培养细菌检出率更高,应该在人工关节感染患者中常规应用。Objective To compare the detection rates of bacteria by culture in BACTEC Plus/F bottles and agar plate.Methods Periprosthetic tissue specimens and synovial fluid were obtained during revision from 58 patients with clinical diagnosed prosthetic-joint infections.Samples were examined by microbiological culture over a 14-day period,and the tissues were subjected to quick frozen section pathologic examination.Results For periprosthetic tissue samples,the sensitivities of BACTEC Plus/F bottle and conventional agar plate cultures for the diagnosis of prosthetic-joint infections were 79.3% and 50%(P=0.005);For synovial fluid samples,the sensitivities of BACTEC Plus/F bottle and conventional agar plate cultures were 74.1% and 45.1%(P=0.003).Overall,culture in BACTEC Plus/F bottles detected significantly fewer contaminants than culture on agar plates(1 versus 6 contaminants,P=0.041).Conclusions These results indicate a superior performance of BACTEC Plus/F bottle cultures over conventional agar plate cultures for the detection of pathogens in patients with prosthetic-joint infections.
