胫腓骨Ⅲ度开放性骨折的外支架固定与组织缺损修复

摘　　要：目的探讨胫腓骨Ⅲ度开放性骨折外支架固定与组织缺损修复的临床疗效。方法对56例(59侧)胫腓骨Ⅲ度开放性骨折彻底清创,胫骨骨折采用单臂外支架固定。适当时机用各种邻近组织瓣修复软组织缺损,并对骨缺损充分植骨。结果小腿创面均治愈。54例(56侧)获得随访,时间9～24(18±8)个月。骨折均愈合,其中2例畸形愈合。钉道感染8例,局部换药治愈。结论胫腓骨Ⅲ度开放性骨折的外支架固定与组织缺损适时修复,能有效地控制局部感染,利于创面与骨愈合。Objective To discuss the clinical curative effect of external fixation and tissue repair for tibiofibular Ⅲ degree open fractures.Methods 56 cases（59 sides） with tibiofibular Ⅲ degree open fractures were retrospectively analyzed.All tibial fractures were treated with single-arm external fixation.Adequate graft was used for bone defects.Proper adjacent tissue flap was used to repair soft tissue defects at the moment.Results All the wounds healed.54 cases（56 sides） were followed up for 9～24（18±8）months.All of the fractures were healed,including malunion in 2 cases.8 got pin track infection and healed after dress changing.Conclusions The treatment with external fixation and tissue repair at the right time for tibiofibular Ⅲ degree open fractures can effectively control the local infection,which will help bone healing.
