摘  要:目的探讨肝细胞癌(hepatocellular carcinoma,HCC)CT灌注成像参数与血管内皮生长因子(vascular endothelial growth factor,VEGF)的相关性。方法以拟行外科手术切除的HCC19例为研究对象,术前采用64层螺旋CT扫描仪进行肝脏CT灌注扫描,采用斜率法计算CT灌注参数,包括肝动脉灌注量(hepatic arterial perfusion,HAP)、门静脉灌注量(hepatic portal perfusion,HPP)、总肝灌注量(total liverperfusion,TLP)和肝动脉灌注指数(hepatic arterial perfusionindex,HAPI)。收集外科手术后肿瘤组织标本,采用免疫组织化学染色检测肿瘤组织VEGF表达情况,分析肿瘤组织血流灌注参数与VEGF的相关性。结果肝癌组织CT灌注参数HAP与VEGF表达显著相关,HPP、TLP和HAPI与VEGF表达无相关性。结论CT灌注成像可用于活体评价肿瘤血管生成。Objective To study correlation between tumor perfusion and vascular endothelial growth factor(VEGF) expression in hepatocellular carcinoma(HCC).Methods Nineteen patients with HCC were performed multidetector computed tomography perfusion imaging before surgical excision.The CT perfusion parameters,such as hepatic arterial perfusion(HAP),hepatic portal perfusion(HPP),total liver perfusion(TLP) and hepatic arterial perfusion index(HAPI),were calculated by using slope method.The VEGF expression in paraffin-embedded tumor specimens of nineteen patients were evaluated by using immunohistochemical method,and the correlation between tumor perfusion and vascular endothelial growth factor expression in HCC was studied.Results Correlation analysis revealed that HAP was positively related to VEGF expression(r = 0.488,p = 0.034,respectively),however,the HPP,TLP and HAPI in tumors were not related to VEGF expression.Conclusion Perfusion CT can provide quantitative information about liver tumors,and this technique has a potential to evaluate the angiogenesis of liver tumors in vivo.
