甲氨蝶呤两种给药途径对治疗未破裂异位妊娠的观察

摘　　要：目的探讨甲氨蝶呤(MTX)以两种不同给药途径治疗未破裂异位妊娠的疗效.方法对113例未破裂异位妊娠患者,用MTX进行治疗,根据不同给药途径随机分两组,A组71例为MTX单次肌注给药,B组42例为MTX经阴道超声引导下异位妊娠囊内注药,对两组的疗效及毒副反应进行统计学分析.结果MTX治疗总体成功率80.53%,A组治愈52例,19例改为手术,成功率73.24%,B组治愈39例,3例改为手术,成功率92.86%,差异有统计学意义(P<0.05);血β-HCG值降至正常的时间:A组(29.8±3.7)d,B组(25.3±4.8)d,差异有统计学意义;A组16例出现MTX副反应,发生率占22.53%,B组3例出现副反应,占7.14%,差异有统计学意义(P<0.05).结论阴道超声引导下异位妊娠囊内注药治疗未破裂型异位妊娠成功率高,显效迅速,不良反应率低,值得选择.Objective To compare the effect of methotrexate in ectopic pregnancy treatment by two different methods.Methods 113 cases with ectopic pregnancy were divided into A and B group.In group A,71 cases were treated with methotrexateby muscle injection;In group B,42 cases were treated with methotrexate by injection guided under transvaginal-ultrasound.Results The total successful rate was 80.53%.73.24% in group A and 92.86% in group B.In group B,the level of β-HCG in blood spent less time to normal than it was in group A and the damage was milder in group B than it was in group A.The effect in group B was significantly better than in group A.Conclusion Methotrexate injection guided under transvaginal-ultrasound in ectopic pregnancy shows higher effect and lmilder damage.It has good value to spread.
