生物反馈改善银屑病患者自主神经功能的研究

摘　　要：１９９０年我们采用生物反馈方法治疗寻常型银屑病患者３１例，总有效率为８４％。最近我们运用规范化负荷诱发性心率变异性（ＨＲＶ）研究方法证实了银屑病患者的副交感神经张力不足和交感神经兴奋性低下，也即表现为自主神经功能全面降低。为研究生物反馈对银屑病的治疗机理是否与改善自主神经功能状态有关，我们设计了本课题。对象和方法：４１例寻常型银屑病患者，２６例有明显精神因素诱发或加重，占６３．４１％；正常人３０名；正常人均选择无器质性病变，无明显心理远负荷。使用“新兴”Ｈｏｌｔｅｒ记录盒和ＨＲＶ分析软件，进行诱发性规范化负荷的ＨＲＶ检查。ＨＲＶ分析是通过心电图中ＲＲ间期变化规律，获得在生物反馈放松训和运动负荷下ＲＲ间期的变动值（ΔＴＲＲ）、相邻ＲＲ间期差的均方根的均数ｒＭＳＡＳＤ以及ＲＲ间期的Ｌｏｒｅｎｚ散点图等，用以揭示自主神经功能调节状态。结果及讨论：（１）在生物反馈放松训练时，无论是银屑病患者组或正常人对照组的ｒＭＳＡＳＤ和ＴＲＲ数值均明显高于生物反馈前，差异有高度显示著性（Ｐ＜００１）。此结果表明在生物反馈放松时副交感神经张力均有所提高。患者组在生物反馈是时ｒＭＳＡＳＤ和ＴＲＲ的均值已接近正常人对照组生物反馈前状?Since 1990,we have adopted the biofeedback method to treat 31 patients with psoriasis vulgaris with a total effective ratio of 84%.Recently,we have confirmed the low level of psoriatic patients’sympathetic nerve excitability and parasympathetic nerve tension by applying the evoked HRV standard successive load method,which also indicates the general decrease in the autonomic nerve functions. In order to study whether the treatment mechanism of psoriatic patienst by biofeedback method is related to the improvement of autonomic nerve functions,we designed the presente theme Objects and Methods:41 patients with psoriasis vulgaris patients have been selected for the study. Twenty-six cases (63.41%) suffered obvious illness aggravation due to psychological factor. At the same time,30 healthy volunteers without organic lesion and obvious psychological load were selected as control. We proceed with HRV check on these people under evoked standard load,using‘Xin Xing’Holter recorder box and HRV analytical software. The HRV analysis can be attained through alternated quantity (ΔT RR ) of RR intervals under biofeedback relax training and motion load,the average of mean square root of successive RR interval, Lorenz scattered pattern in RR interval,etc.These data reveal autonomic regulation function state. Results and discussion: (1)During biofeedback process,the values of rMSAS and T RR of of both psoriatic group and healthy control were obviously higher than those before biofeedback relax treatment. The difference was very significant ( P <0.01) The results showed that parasympathetic nerve tension was invaribly raised and the average values of rMSASD and TRR of patient group at biofeedback stage were similar to those of healthy controls before biofeedback treatment. (2) The average values of rMSASD 、T RR and T RR were generally raised after biofeedback treatment in contrast with before.
