经皮肾微造瘘双频双脉冲激光碎石治疗肾结石的疗效

摘　　要：目的探讨双频双脉冲U100（Nd：YAG）激光在经皮肾微造瘘粉碎肾结石中的疗效及安全性。方法应用双频双脉冲U100（Nd：YAG）激光经经皮肾微造瘘治疗肾结石202例,男性122例,女性80例,年龄16~79岁,平均45.8岁。单发性结石72例,多发性结石55例,鹿角结石75例,双侧肾结石34例。结果本组202例均未发生输尿管、膀胱、尿道穿孔和损伤等并发症,且均一期手术取石,其中一通道取石159例,二通道取石33例,三通道取石10例。138例为一次性取石,64例二次以上取石,182例结石全部取净,余20例有少许残石者,术后行ESWL,结石基本排净。肾结石碎石成功率为100%（202/202）。结论双频双脉冲U100（Nd：YAG）激光能迅速、高效地粉碎肾结石,并发症少,疗效确切,是目前治疗泌尿系结石的一种理想方法。Objective To evaluate the clinical effect of U100 frequency-doubled-double-pulse Nd：YAG laser lithotripsy in the treatment of renal stone. Methods 202 patients with different kinds of renal stones were treated by U100 frequency-doubled-double-pulse Nd：YAG laser lithotripsy. Results Percutaneous renal stone fistulization were successful in all the patients.Renal stones were fragmented and removed completely in 182 patients and the other 22 patients received ESWL to remove the remained stones.The total stone clearance rate was 100%.There was no severe postoperative complication in all the patients. Conclusion U100 frequency-doubled-double-pulse Nd：YAG laser lithotripsy has the advantages of efficiently breaking the stone and less side effect.It is a good choice for the treatment of urolithiasis.
