摘 要:目的探讨胃袖状切除术的减肥作用及其机制。方法通过高脂饮食诱导SD肥胖大鼠,然后随机分为胃袖状切除(SG)组和假手术(SS)组。术后连续观察8周,比较两组术后体重、摄食和外周血Ghrelin、GLP-1、PYY3-36浓度的变化。结果SG组术后摄食量明显下降(P<0.05),体重连续3周持续下降,然后缓慢回升,至第8周时体重仍低于术前,外周血Ghrelin浓度下降,GLP-1和PYY3-36浓度升高(P<0.05),随着时间延长,血Ghrelin浓度逐渐升高,GLP-1和PYY3-36浓度逐渐回落。SS组术后食量、血Ghrelin、GLP-1和PYY3-36浓度无明显变化(P>0.05),8周后体重明显高于术前(P<0.05)。结论肥胖大鼠胃袖状切除术减重效果确切,术后Ghrelin浓度下降,GLP-1及PYY3-36浓度升高致使摄食量减少可能是其减肥的主要作用机制。Objective To investigate the weight-lowering mechanism of sleeve gastrectomy(SG).Methods SD rats were fed on high-fat diet to induce obesity and randomly divided into experimental group(SG) and control group(sham operation).During a follow-up period of 8 weeks,Rats' body weight,food intake and peripheral blood levels of Ghrelin,GLP-1,PYY3-36 were checked.Results In the experimental SG model,food intake was decreased(P0.05);Postoperative weight continued to decline for three weeks and then slowly recover.Eight weeks later,body weight was still lower than that before the operation;Ghrelin was decreased,and GLP-1 and PYY3-36 increased(P0.05).Over time,Ghrelin was gradually increased,and GLP-1 and PYY3-36 gradually declined.In the control group,postoperative weight was markedly increased after decline for one week.Eight weeks later,weight was significantly higher than that before operation;Food intake,Ghrelin,GLP-1 and PYY3-36 had no significant change(P0.05).Conclusion The weight-lowering effects of SG in DIO rats are obvious.The postoperatively decreased Ghrelin,and increased GLP-1 and PYY3-36 are the major mechanism.
