摘 要:目的:比较腹腔镜手术术前、术中、术后β2-微球蛋白、肾脏彩色多普勒超声的变化,探讨β2-微球蛋白、彩色多普勒超声在腹腔镜术中评估肾功能的应用价值。方法:腹腔镜胆囊切除术(laparoscopic cholecystectomy,LC)患者83例,分别在术前和气腹后30min、90min、术后第1天、第3天抽取空腹静脉血送检及行彩色多普勒超声检查,检查项目为BUN、Scr、β2-微球蛋白及肾脏的血流指数。结果:与气腹前相比,β2-微球蛋白在气腹后30min、90min均升高,差异有统计学意义,术后第1天、第3天差异无统计学意义;主肾动脉阻力指数在气腹后30min、90min均升高,差异有统计学意义,术后第1天、第3天差异无统计学意义;BUN、Scr在气腹后90min均升高,差异有统计学意义,气腹后30min、术后第1天、第3天差异无统计学意义。结论:腹腔镜术中β2-微球蛋白、彩色多普勒超声可用于早期评估肾功能。Objective:To investigate the value of β2-microglobulin and color Doppler ultrasonography for evaluation of renal function in laparoscopic surgery by comparing the change of β2-microglobulin and color Doppler ultrasound data before,during and after laparoscopic operation.Methods:Eighty-three cases who underwent laparoscopic cholecystectomy (LC) were selected.The fasting venous blood samples were collected before operation,30min and 90min after pneumoperitoneum 1st and 3rd day after operation.Meanwhile,color Doppler ultrasounds were also applied at the time points mentioned above.The blood flow index of kidney,BUN,Scr and β2-microglobulin were examined.Results:Compared with the data before the operation,the β2-microglobulin were much higher at 30min and 90min after pneumoperitoneum,the differences had statistical significance.However,the differences in same kind of data at 1st and 3rd day after operation wasn' t statistically significant.The main renal arterial resistive index both increased at 30min and 90min after pneumoperitoneum,the differences also had statistical significance.There wasn' t statistical difference between the data of 1st and 3rd day after operation.Both BUN and Scr increased 90min after pneumoperitoneum,the differences had statistical significance.But the detection results had no significance at 30min after pneumoperitoneum,1st and 3rd day after operation.Conclusions:In laparoscopic surgery,β2-microglobulin measurement and color Doppler ultrasonography are appropriate methods to evaluate the renal function in early stage.
