激素性股骨头坏死FicatⅠ期优化方案的临床研究

摘　　要：目的:对激素性股骨头坏死FicatⅠ期各种治疗措施进行评价,找出该期的最佳治疗方案。方法:在文献研究的基础上,筛选出几种疗效较佳、远期随访塌陷率最低的治疗方案,作为临床研究的基础。共收集2006年1月至2008年12月课题组所在医院治疗的激素性股骨头坏死FicatⅠ期患者82例98髋;其中男性55例,女性27例;平均年龄36岁。对比临床疗效及手术疗法所致创伤和康复情况。结果:FicatⅠ期治疗方案中,多孔细针能有效减压,增加静脉排出,改善血液循环,促进组织修复;微创、简单。自体骨髓干细胞移植术具有多分化潜能,可向成骨细胞转化,促进成骨作用。结论:FicatⅠ期治疗优化方案为经皮穿孔减压干细胞移植。Objective：evaluating the different therapeutic measure for the Steroid-induced Necrosis of Femoral head（SANFH） FicatⅠstage,our aim is to find the best proposal for treating it.Methods：based on the literature research which counted the short term effect and long-term effect,we filtered some therapeutic measure used in the SANFH FicatⅠstage for the Clinical study.82 patients（98 hip,male：55 cases,female：27 cases,average age：38 years old） which were treated from 2006.1-2008.12 in the hospital that the researcher worked in were collected.compared their curative effect,wound and recovery from operative treatment.Results：In the SANFH Ficat Ⅰ stage,we found that multi-hole small pin decompression can decompress effectively,increase haemostasis discharge,improve blood circulation,promote organization restoration,mini-ravage and easy.bone marrow stem cells（BMCs） has multi-splitting potential,can transfer to osteoblast,and promote osteogensis.Conclusion：The optimization scheme of Ficat Ⅰ stage：the percutaneous perforation stem cell transplantation.
