关节镜下微创治疗胫骨平台骨折疗效分析

摘　　要：目的:探讨在关节镜辅助下微创治疗胫骨平台骨折的方法及疗效。方法:2001-02-2009-02收治SchatzkerⅠ～Ⅲ型胫骨平台骨折患者40例,常规进行膝关节镜检查,对合并伤进行相应的处理后,在关节镜与型臂线机辅助下微创进行骨折复位、植骨及固定,术后早期行患肢功能锻炼。结果:40例患者经6～24个月随访,平均随访时间18个月,在3～4个月骨性愈合,未出现塌陷、移位、膝关节畸形等。根据膝关节以HNSS评分标准,优27例,良9例,中4例,优良率为90%。结论:关节镜辅助下微创治疗胫骨平台骨折,具有创伤小,同时处理关节内合并伤,获得良好复位及术后功能恢复快、效果好的优点。术中对复位判断更为准确,降低术后畸形愈合率,为胫骨平台骨折手术的较理想方法。Objective：To investigate clinical results of the arthrocopic technique in the treatment of tibial plateau fractures.Methods：A total of 40 cases of tibial plateau fractures,consisting of 11 cases of type I,18 of II,11 of III according to Schatzker classification,were treated with reduction and screw fixation assisted arthroscopically over a span from 2001 to 2009.Theassociated intra articular pathology were treated simultaneously.All of the cases were followed up 6 to 24 months and post-operational complications and function recovery were investigated.Results：All of the fractures healed within three months post-operationally without complication.The rate of excellent and good results was 90% according to HNSS assessment criterion.Conclusion：The reduction and fixation with assistance of arthroscope for the tibial plateau fracture has the advantages of minimal injury,safe operation,accurate reduction and fixation,early recovery and convenient treatment of the associated intraarticular pathology at the same time.
