摘 要:目的探讨靶向超声介导VEGF165cDNA、抗ICAM-1单克隆抗体及两者协同作用于受损心肌的有效性及可行性。方法将80只心肌缺血的雄性SD大鼠随机分为四组:第一组采用超声击破微泡造影剂的方法将VEGF165cDNA基因及抗ICAM1单克隆抗体混合物定向于受损心肌;第二组采用超声击破微泡造影剂的方法将VEGF165cDNA基因定向于受损心肌;第三组采用超声击破微泡造影剂的方法将抗ICAM-1单克隆抗体定向于受损心肌;第四组为对照组只注射生理盐水1ml,不采用超声波照射。每组分别于24h和14d后处死10只。处死后分别行坏死区中性粒细胞记数,梗死区测定,免疫组织化学检测缺血心肌中的VEGF蛋白表达,缺血坏死心肌区域毛细血管记数。结果第一组大鼠心肌缺血治疗效果较第二、第三组效果显著,第一组中性粒细胞浸润以及心肌梗死区域均明显低于其他各组,VEGF蛋白表达以及毛细血管记数明显高于其他三组。结论采用超声触发破坏含靶基因及抗体的微泡造影剂的方式,可明显对抗心肌再损伤,促进缺血心肌的新生血管再生,改善心肌缺血,且两者协同作用比单独作用效果更佳。Objective To study the cardioprotective effect of VEGF165eDNA gene and monoclonal antibody to interceller adhesion molecule-1 (ICAM 1) delivering and two coordination to myocardial ischemia-reperfusion injury of rats by ultrasound targeted microbubble destructive. Methods Eighty male SD rat models of myocardial ischemia-reperfusion injury (I/R) were divided into 4 groups. Using microbubble contrast agent destructive methods,the first group was infused VEGF165 cDNA and ICAM-1, the second group was infused VEGF165 eDNA, the third group was infused ICAM-1, the fourth group was used as control group. Each group was divided into two groups again. One group(24 h group) was killed after 24 hours and another group(14 d group) was killed after 14 day, the expression of VEGF protein was accessed by immunohistochemical technique. Microvascular density (MVD) and polymorphonuclears(PMN) were counted in the infarcted area. The area of myocardial necrosis was measured. Results The infiltration of PMN and the area of myocardial necrosis received ultrasound-mediated in the 24 h group of the first group was lower than other groups. VEGF protein expression and MVD in 14 d group of the first group was higher than other groups. Conclusions Using targeted ultrasound may enhance obviously concentration of VEGF165 eDNA gene and ICAM-1 in myocardial ischemia. And the synergy of VEGF165 eDNA gene and ICAM-1 is better than the single.
