摘 要:目的:初步探讨2型糖尿病患者血尿酸水平与肌酐清除率的关系。方法:选择华山医院住院的2型糖尿病患者163例,应用Cockcroft-Gault公式估计肌酐清除率,分析2型糖尿病患者血尿酸与肌酐清除率的关系。结果:(1)高尿酸血症组体质量指数、尿素氮、肌酐及尿酸水平均明显高于非高尿酸组(P<0.05),而糖化血红蛋白(HbA1c)及肌酐清除率均明显低于非高尿酸组(P<0.05);(2)血尿酸水平与肌酐清除率(相关系数:-0.161,P=0.043)及HbA1c(相关系数:-0.299,P<0.001)均显著负相关,HbA1c与肌酐清除率显著正相关(相关系数:0.184,P=0.032);(3)逐步Logistic回归分析显示,低HbA1c及高BMI为高尿酸血症的独立危险因素,肌酐清除率与血尿酸有负相关趋势。结论:2型糖尿病者血尿酸水平与HbA1c、肌酐清除率呈显著负相关,肌酐清除率的增高可能是2型糖尿病者血尿酸降低的原因之一。Objective:To explore the association of serum uric acid level with creatinine clearance rate in type 2 diabetic patients. Methods:One hundred sixty-three in-patients with type 2 diabetes in Huashan hospital were selected. Creatinine clearance rate was estimated by Cockcroft-Gault equation. The correlation analysis and logistic regression analysis were employed to test the relationship of serum uric acid,HbA1c and creatinine clearance rate. Results:(1) Body mass index,blood urea nitrogen,serum creatinine and serum uric acid in hyperuricemia subgroup were significantly higher than those in non-hyperuricemia subgroup. HbA1c and creatinine clearance rate in hyperuricemia subgroup were lower than those in non-hyperuricemia subgroup. (2) Seurm uric acid was negatively correlated with creatinine clearance rate (correlation coefficient:-0.161,P=0.043) and HbA1c(correlation coefficient:-0.299,P0.001). HbA1c was correlated with creatinine clearance rate (correlation coefficient:0.184,P=0.032). (3) Obesity and low HbA1c were risk factors of hyperuricemia by stepwise Logistic regression,and negative correlation tendency was also shown between serum uric acid and creatinine clearance rate. Conclusion:Seurm uric acid was negatively correlated with creatinine clearance rate and HbA1c in type 2 diabetic patients,and the increased creatinine clearance rate might contribute to the fall of serum uric acid level.
