厄贝沙坦对原发性高血压患者血清脂联素水平的影响

摘　　要：目的观察厄贝沙坦对原发性高血压患者血清脂联素(adiponectin,APN)水平的影响。方法选择原发性高血压2级及3级患者共65例,分观察组(32例)和对照组(33例),分别给予厄贝沙坦和苯磺酸左旋氨氯地平片治疗,病程12周,检测治疗前后患者APN、收缩压(SBP)、舒张压(DBP)、血清总胆固醇(TC)、三酰甘油(TG)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)、空腹血糖(FBS),并计算体质指数(BMI)。分析药物对APN的影响。结果(1)两组收缩压、舒张压治疗后均较治疗前明显下降(P<0.01),两组之间差异无统计学意义(P>0.05);(2)治疗组APN治疗前(1.95±0.88)μg/mL,治疗后上升至(4.27±3.23)μg/mL,治疗前后差异有统计学意义(P<0.01),与对照组比较差异有统计学意义(P<0.01)。结论厄贝沙坦具有升高APN的作用,提示厄贝沙坦可能通过增加APN水平,改善胰岛素抵抗(IR)来达到一定的降压效果。Objective To study the effects of irbesartan on serum adiponectin level in patients with essential hy- pertension. Methods Sixty five patients with moderate to severe essential hypertension were included and randomly di- vided into observation group （32 cases） and control group （33 cases ）. Subjects in observation group and control group were treated with Irbesartan and acid L- amlodipine tablets for 12 weeks, respectively. Systolic blood pressure （SBP）, diastolic blood pressure （DBP） , serum total cholesterol （TC） , triglycerides （TG） , high- density lipoprotein cholesterol （HDL） , low - density lipoprotein cholesterol （LDL） , fasting blood sugar （FBS） and calculated body mass index （BMI） were measured at baseline and after treatment for comparison. Effects of drugs on APN were also assessed. Results （ 1 ） Systolic blood pressure and diastolic blood pressure in both groups decreased significantly after treatment （P 〈 0. 01 ） , but there was no statistically significant difference between the two groups （ P 〉 0. 05 ）. （ 2 ） In observation group, APN in- creased significantly after treatment （4. 27 ± 3.23 vs 1.95 ±0. 88, P 〈0. 01 ）. Meanwhile, APN was significantly higher in observation group than control group after treatment （ P 〈 0. 01 ）. Conclusion Irbesartan may lower blood pressure via increasing APN level and improving insulin resistance （IR）.
