血管炎患者血浆可溶性CD146的意义

摘　　要：目的检测可溶性黏附分子CD146(sCD146)在不同血管炎患者血浆中的水平,分析其与血管炎病情活动的关系及发病机制中的可能意义。方法夹心ELISA法检测29例系统性血管炎(8例白塞病、10例韦格纳肉芽肿、8例显微镜下多血管炎、3例变应性肉芽肿性血管炎)、20例系统性红斑狼疮(SLE)患者和20例健康体检查(对照组)的sCD146水平,分析其与病程、年龄、血清肌酐(Scr)、血沉(ESR)、C-反应蛋白(CRP)、抗中性粒细胞胞浆抗体(ANCA)和伯明翰血管炎活动指数(BVAS)的相关性。观察其中15例患者活动期及好转期sCD146的动态变化。结果显微镜下多血管炎(MPA)患者组血浆中sCD146的水平为(403.2±144.1)ng/mL,较对照组明显升高(P<0.01),其水平与年龄和Scr相关,与炎症指标和BVAS无关;其他各组与正常人相比差异均无显著性(P>0.05)。活动期及好转期sCD146的水平分别为(336.1±124.2)ng/mL和(414.3±147.4)ng/mL,差异有显著性(P=0.003)。结论MPA患者sCD146的水平升高,与年龄和Scr相关,也许反映了广泛而严重的微、小血管内皮细胞(EC)损伤。Objective To examine the concentrations of plasma soluble CD146 （sCD146） in different systemic vasculitis patients, and to analyze the correlation between sCD146 and clinical activity, thus to evaluate its possible role in the pathogenesis of systemic vasculitis. Methods Sandwich ELISA was used to measure plasma sCD146 in 29 patients with systemic vasculitis （8 with Behcet＇s disease, 10 with Wegener＇s granulomatosis, 8 with Microscopic polyangiitis, 3 with Churg- Strous syndrome） and 20 patients with SLE. Correlations between sCD146 and disease activity variables were analyzed. Furthermore, dynamic observation of sCD146 was performed in 15 patients during active and improved phase. Results The mean concentration of sCD146 in MPA patients was （403.2± 144. 1 ） ng/mL, significantly higher than that of controls （P 〈0. 01, matched with age and gender）. Concentration of sCD146 was significantly correlated with age and serum creatinine, but not with inflammation variables and Birmingham vasculitis activity score （BVAS）. There was no significant difference between the other groups and controls （ P 〉 0. 05 ）. Concentrations of sCD146 in active and improved phase were （336. 1 ± 124. 2） ng/mL and （414. 3± 147.4） ng/mL, respectively, with significant increase in improved phase（ P = 0. 003）. Conclusion Concentration of sCD146 in patients with MPA is significantly elevated and correlates with age and serum creatinine. It may reflect the damage in vascular endothelial ceils of small vessels.
