摘　　要：目的探讨原发性甲状旁腺功能亢进(PHPT)微创甲状旁腺切除术测定甲状旁腺激素(PTH)的意义。方法2004年1月—2009年11月11例PHPT患者行微创甲状旁腺手术,分别测定麻醉后手术前、病变甲状旁腺切除后10 min、术后第1天、术后7天、术后6个月的血PTH水平,并检测血钙及血磷的变化情况。结果11例患者甲状旁腺病变切除后10 min血PTH较术前下降>50%,证实高功能甲状旁腺组织全部切除。10例患者术后6月复查血钙、血磷均正常,9例PTH正常,1例略升高。结论在原发性甲状旁腺功能亢进微创手术中,PTH测定是确定是否全部切除病变甲状旁腺组织的可靠指标。Objective To investigate intra-operative PTH monitoring in the minimally invasive parathyroidectomy for primary hyperparathyroidism（PHPT）.Methods Retrospectively analyzed 11 cases of PHPT with minimally invasive parathyroidectomy from January 2004 to November 2009.PTH,serum calcium and phosphorus were measured before operation,10 mins,1 day,7 days and 6 months after parathyroidectomy respectively.Results 10 minutes after operation,PTH in all of the 11 patients declined more than 50%compared with pre-operation,which confirmed the complete resection of the hyper-functioning tissue.6 months after operation,10 patients had normal serum calcium and phosphorus level,9 had normal PTH level,only one patient with mild PTH elevation.Conclusion Intra-operative PTH monitoring is a reliable index which can confirm whether the hyper-functioning parathyroid tissue was completely resected or not.
