带蒂真皮下血管网超薄皮瓣在手烧伤后畸形修复中的应用

摘　　要：[目的]探讨手烧伤后畸形修复的新方法,评价带蒂真皮下血管网超薄皮瓣的临床应用价值。[方法]应用腹部带蒂真皮下血管网超薄皮瓣治疗手部烧伤后畸形12例,术后7～9d断蒂。[结果]12例带蒂真皮下血管网超薄皮瓣全部存活,单块皮瓣的最大面积为12cm×11cm,最小为11cm×9cm。术后断蒂时间7～9d,术后随访6个月～2年,所有患者转移皮肤质地、颜色与手部原来皮肤相仿,形态恢复及运动功能恢复满意,感觉功能稍差。[结论]带蒂真皮下血管网超薄皮瓣设计、操作简单、安全,伤肢固定时间较短,恢复效果较好。[Objective] To investigate a new method of repair for the post-burn hand deformities, to evaluate clinical application value of pedicled thin skin flap with subdermal vascular net. [Methods] The abdominal pedicled subdermal vascular net thin skin flap was used for treatment of 12 cases of hand deformities after being burnt. [Results] 12 cases with pedicled thin skin flap with subdermal vascular net all survived, the largest single piece of flap area was 12 cm × 11 cm, a mini- mum was 11 cm × 9cm. The time of cutting off pedicles was 7-9d, follow up time was 6 months to 2 years. All patients’ skin texture, skin color of hands were similar to the original hands, recoveries of shape and function were satisfactory, sensory function was not good. [Conclusion] The pedicle skin flap with subdermal vascular net thin design has simple and safe operation, and short time in fixation time of limbs and better recovery effect, which is worthy for being popularized.
