摘  要:目的探讨实时三平面(RT-3PE)定量组织速度成像(QTVI)与应变率成像(SRI)技术评价高血压病左室舒张功能的临床价值。方法38例高血压病患者(高血压病组)和40例正常人(正常对照组),应用RT-3PEQTVI技术采集心尖四腔观三平面图像,取左室6个壁的基底段和中间段,应用定量组织速度成像技术测量各节段舒张期峰值速度(Ve、Va),应用SRI获得各节段舒张期峰值应变率(SRe、SRa),计算Ve/Va及SRe/SRa。用血流多普勒法测定二尖瓣口血流峰值速度(E、A)及比率(E/A),进行分析、比较。结果与正常对照组比较,高血压病组E/A、SRe、SRe/SRa和Ve、Ve/Va明显降低,两组SRe与Ve、SRe/SRa与Ve/Va有高度相关性。结论实时RT-3PEQTVI结合SRI能准确地测量高血压病患者舒张期左室壁运动速度及左室心肌应变率,为无创检测高血压病患者左室舒张功能提供了新方法。Objective To explore the clinical value of real-time three-plane(RT-3pE) quantitative tissue velocity imaging(QTVI) and strain rate imaging(SRI) which evaluating left ventricular diastolic function in patients with essential hypertension.Methods Thirty-eight patients with essential hypertension and forty healthy subjects were detected by RT-3PE QTVI.The QTVI of apical four-chamber view were obtained.The 6 walls of left ventricle were divided into basal and middle segments.Early diastolic velocity (Ve) and late diastolic velocity (Va) at six sites of mitral annulus were measured using QTVI.Early diastolic strain rate (SRe),late diastolic strain rain (SRa) at middle and basal ventricule were measured using strain rate imaging(SRI),then SRe/SRa and Ve/Va ratio were calculated.Peak flow velocity(E,A)and ratio (E/A) of mitral valve orifice blood flow in diastolic phase were measured using the frequency spectrum.These data were analyzed and compared.Results Compared with the control group,the parameters of E/A,SRe,SRe/SRa,and Ve,Ve/Va were significantly reduced.There were a high correlation between SRe and Ve,a high correlation between SRe/SRa and Ve/Va.The ratios of Ve/Va and SRe/SRa were more sensitive targets of left ventricular diastolic function than E/A in essential hypertension.Conclusion RT-3PE QTVI and SRI can quantify diastolic velocity and diastolic strain rate on walls of left ventricular in patients with essential hypertension properly.It is a new noninvasive method in assessment of left ventricular diastolic function in essential hypertension.
