视网膜变性伴有圆形裂孔激光治疗与否的长期观察

摘　　要：目的讨论圆形裂孔伴有视网膜变性激光治疗的效果。方法回顾性分析视网膜变性伴有圆形裂孔患者256例（272只眼）接受氩激光治疗，同期192例（198只眼）未接受激光的自然观察病例做对照。分析其在追踪观察时间（36．2±8．6）个月内的视网膜脱离发生率、视网膜脱离发生时间以及原裂孔与视网膜脱离发生的关系。结果两组视网膜脱离发生率分别为12．6％和13．1％，对比差异无统计学意义（P〉0．05）。激光治疗组6个月内视网膜脱离发生率为35．2％，对照组为19．2；两组对比差异有统计学意义（P〈0．05）。激光治疗组17．7％的患者为原裂孔引发的视网膜脱离，对照组38．4％是原裂孔引起，两组对比差异有统计学意义（P〈0．05）。结论视网膜变性伴有圆形裂孔自然病例视网膜脱离的发生率与激光治疗并无差别，而且激光治疗组在6个月内视网膜脱离发生率明显高于对照组。大多数新裂孔可能与激光有关，对视网膜变性伴有圆形裂孔激光治疗应持慎重态度。Objective To explore the therapeutic effect of argon laser therapy of retinal degeneration with oval-shaped retinal holes.Methods A total of 256 retinal degeneration patients （272 eyes） with retinal holes who underwent the argon laser therapy in our department were retrospectively analyzed,which was compared with the natural course of retinal degeneration with oval-shaped retinal holes of 192 patients （ 198 eyes） without argon laser therapy.The retinal detachment rate, the time of the retinal detachment developed and the relationship between retinal detachment were analyzed during the 36.2 ± 8.6 months follow-up time.Results The retinal detachment rate of two groups was about 12.6% and 13.1% respectively.The difference was not statistically significant.However, at the first 6 months the retinal detachment rate of about 35.2% and 9.2% respectively.Difference between the two groups was statistically significant （P〈0.05）.The retinal detachment was 17.7% in the original retinal holes of the patients and 38.4% who has underwent the laser therapy group.Difference between the two groups was statistically significant （P〈0.05）.Conclusions The difference between the two groups of the patients either received laser therapy or not is not statistically significant in the retinal degeneration with oval-shaped retinal holes.It is easy to see that the number of retinal detachment in the group of patients who have underwent the laser therapy is more than the other group at the fist 3 months.The laser therapy may be the cause of the new retinal holes.So laser therapy on the retinal degeneration with oval-shaped retinal hole needs to be cautious.
