5-FU对结肠癌转移灶抑制率与耐药相关因子表达关系的实验研究

摘　　要：目的探讨结肠癌原发灶及淋巴结转移灶对5-氟尿嘧啶(5-FU)的体外化疗药敏性及其与多种耐药因子表达的关系。方法对48例结肠癌原发灶、转移淋巴结肿瘤细胞进行体外化疗药敏性实验,并行P-gp,GST-π,p53,survivin和Bcl-2,Bax免疫组化染色。结果5-FU对淋巴结转移灶肿瘤细胞的抑制率高于原发灶(P<0.05)。P-gp和Bax在肿瘤淋巴结转移灶中表达程度高于原发灶(均P<0.05),Bcl-2在淋巴结转移灶中的表达明显下降(P<0.05)。结肠癌原发灶中5-FU药物抑制率与P-gp的表达呈负相关(P<0.05);转移淋巴结中药物抑制率与Bax表达呈正相关,与Bcl-2表达呈负相关(均P<0.05)。结论影响结肠癌淋巴结转移灶中5-FU药敏性的主要耐药相关因子与原发灶不同,术后辅助化疗及逆转结肠癌的多药耐药性应针对淋巴结转移灶进行。Objective To investigate the relationship between expressions of muhidurg resistance associated factors and chemosensitivity of 5 -fluorouracil （ 5 -FU ） in lymph node metastases （ LNMs ） of colon carcinoma. Methods The chemosensitivity to 5-FU was measured by MTT assay, and the expressions of P-gp, GST-＇n, p53 , survivin, Bcl-2 and Bax were determined immunohistochemically in 48 paired primary tumor （ FF ） and LNMs of colon carcinoma. Results The inhibition rates of LNMs cells for 5- FU were higher than that of PT （ P 〈 0.05 ）. The expressions of P-gp and Bax were higher in LNMs than those in PT （ both P 〈 0.05 ） , and lower expression of Bcl-2 was found in LNMs （ P 〈 0. 05 ） . There was statistically negative correlation between inhibition rates and expression of P-gp in PT （ P 〈 0. 05 ） ; Bcl-2 had negative correlation with inhibition rates for 5-FU in LNMs （ P 〈 O. 05 ） , and positive eorrelativity was found between Bax and inhibition rates for 5-FU （ P 〈 0.05 ） . Conclusions The main factors that affect chemosensitivity to 5-FU in colon carcinoma in PT and in LNMs are different. Effective adjuvant chemotherapy after operation and reversion of muhidrug resistance （ MDR ） of colon cancers depends on targeting the metastatic component of the disease.
