聚蛋白多糖酶和基质金属蛋白酶在不同期骨关节炎患者血清中的表达及其意义

摘　　要：目的比较解整链蛋白金属蛋白酶(ADAMTS)4、5和基质金属蛋白酶(MMP)1、3在骨关节炎(OA)患者早期和中晚期血清中的表达,并探讨其在OA患者不同时期的作用和早期诊断意义。方法70例单关节OA患者按照MRIRecht分级标准分为早期OA组(44例)和中晚期OA组(26例),同时设健康对照组(30例)。应用酶联免疫吸附试验(ELISA)检测ADAMTS-4、ADAMTS-5、MMP-1和MMP-3在三组血清中的表达,以ELISA光密度(OD)值代表所测各项在血清中的表达水平。结果早期OA组的ADAMTS-4表达水平高于中晚期OA组;ADAMTS-5、MMP-1和MMP-3在早期OA组的表达水平均低于中晚期OA组(P均<0.05)。而正常对照组血清中仅有少量聚蛋白多糖酶和MMP-1的表达。结论聚蛋白多糖酶在OA早期和中晚期血清中均有高表达,MMP在OA中晚期血清中有高表达,ADAMTS-4有早期诊断意义。Objective To detect expression of ADAMTS-4,ADAMTS-5,MMP-1,MMP-3 in the serum of early and late osteoarthritis（OA） patients,and to explore the different effects of ADAMTS and MMP on OA patients. Methods 70 cases were divided into early OA（44 cases） and late OA（26 cases） groups according to Recht’s standard. The control group involved 30 cases. Expression of ADAMTS-4,ADAMTS-5,MMP-1,MMP-3 in the three groups were investigated by enzyme-linked immunosorbent assay （ELISA） and represented by optical densities （OD）. Results ADAMTS-4 had higher expression in the early OA group than in the late OA group （P〈0. 05）. ADAMTS-5,MMP-1,MMP-3 had lower expression in the early OA group than in the late OA group （P〈0. 05）. Only a few ADAMTS-4,-5 and MMP-1 were expressed in the control group. Conclusion Aggrecanases have high expression in the early and late OA groups and MMPs have high expression in late OA,and ADAMTS-4 has value in the early diagnosis of OA.
