三七透明质酸钠凝胶对家兔椎板切除术后硬膜外瘢痕中胶原成分的影响

摘　　要：目的:探讨三七透明质酸钠凝胶在家兔术后硬膜外瘢痕组织形成过程中对Ⅰ、Ⅲ型胶原表达的影响。方法:大耳白品系家兔96只,体重2.0～2.5kg,6月龄,雌雄各半,按照完全随机方法分为生理盐水组(A)、三七组(B)、透明质酸钠组(C)、三七透明质酸钠凝胶组(D),制作家兔椎板切除模型,分别在硬膜囊周围涂抹生理盐水、三七浓缩液、透明质酸钠、三七透明质酸钠凝胶各0.5ml,各组术后1、2、4、8周取材,Masson染色进行胶原生长情况的形态学观察,原位杂交方法分析Ⅰ、Ⅲ型胶原mRNA的表达。结果:Masson染色中胶原纤维的排列,生理盐水组三七透明质酸钠凝胶组比对照组更加规整。Ⅰ型胶原mRNA表达三七透明质酸钠凝胶组在用药4周时明显低于生理盐水组、透明质酸钠组、三七组(P<0.01);Ⅲ型胶原mRNA表达三七透明质酸钠凝胶组在用药4周时明显高于生理盐水组、透明质酸钠组、三七组(P<0.01)。结论:三七透明质酸钠凝胶对家兔术后硬膜外瘢痕具有改善胶原纤维的排列,降低瘢痕硬度,增加瘢痕弹性的作用。Objectives：To study the effects of Sanqi （三七） qisodium hyaluronate gel on collagen Ⅰand collagen Ⅲ expression in the process of rabbits＇ epidural scar formation after operation. Methods：Ninety six white rabbits with 6 month old, half males and half females,weighted from 2 to 2.5 kg,which were randomly divided into normal saline group （A）,Sanqi （三 七） group（B）,qisodium hyaluronate group（C） and Sanqi （三七） qisodium hyaluronate gel group（D）. The laminectomy of rabbits were performed in group A,B,C D,the duras were surrounded with normal saline,Sanqi （ 三 七 ） liquid,qisodium hyaluronate and Sanqi（三七） qisodium hyaluronate gel respectively. Animals of each group were killed at 1,2,4,8 weeks after operation. Use Masson staining for histological observation of collagen,and in situ hybridization staining for the analysis of collagen Ⅰand collagen Ⅲ expression. Results：In the Masson staining,Sanqi （三七） qisodium hyaluronate gel group was more regular than the control group in the shape of collagen texture. As to the expression of collagen Ⅰ,and Sanqi （三七） qisodium hyaluronate gel group was lower than normal saline group,the Sanqi （三七） group and qisodium hyaluronate group at 4 weeks after using medicine （P0.01）;while the Sanqi （三七） qisodium hyaluronate gel group was higher than normal saline group, Sanqi （三七） group and qisodium hyaluronate group in the collagen Ⅲ expression （P0.01）. Conclusions：Sanqi （三七） qisodium hyaluronate gel could improve collagen＇s arrangement of the rabbit＇s epidural scar after operation,reduce its rigidity and increase flexibility.
