摘  要:目的:通过观察乌司他丁对肝脏移植早期外周血NF-κB、TNF-α和酶学活性的影响,研究乌司他丁对肝脏移植术后早期再灌注损伤的保护作用。方法:收集36例肝硬化晚期患者,随机分为实验组和对照组(各18例),实验组在肝移植时给予乌司他丁30万U+生理盐水10mL静脉滴注,对照组给生理盐水10mL静脉滴注,在供肝恢复血供后1、2、4及6h抽取患者外周血进行肝酶学检查,用Westernblot技术检测外周血中核转录因子κB(NF-κB)P65表达,用双抗体夹心酶联免疫吸附法测定外周血中TNF-α表达水平。结果:和对照组相比,实验组肝移植术后早期各时相点外周血肝酶学指标、NF-κBP65和TNF-α的表达水平明显下降,且差异均有统计学意义(P<0.05)。结论:乌司他丁能抑制肝移植后早期肝脏炎症通路,对肝脏移植后再灌注损伤具有保护作用。Objective: To investigate the effect of urinastatin on the expression levels of NF-κB and TNF-α liver transplantation in patients. Methods: Thirty-six patients of late stage hepatic cirrhosis were randomly divided into two eaquel groups: the experiment group was administrated by urinastatin 300 000 U and saline 10 mL via venous injection during liver transplantation, the control group was administrated 10 mL saline by venous injection at the same time. The blood samples were harvestd at 1, 2, 4 and 6 h after the blood recovery of donated liver. The nuclear factor-κB (NF-κB) P65 relative level of blood was detected by Western blot analysis and the TNF-α level of blood was detected by double antibody Sandwich enzyme linked immunosorbent assay. Serum ALT and AST were also measured. Results: The expression level of serum NF-κB p65, TNF-α in the experiment group were lower than those in the control group, and the difference was significant(P〈0.05). The levels of ALT and AST of peripheral blood showed the same tendency. Conclusion: The inflammatory pathway following liver transplation was attenuated by urinastatin.
