阻塞性睡眠呼吸暂停低通气综合征患者血清瘦素水平与甲状腺素的相关性研究

摘　　要：目的探讨阻塞性睡眠呼吸暂停低通气综合征(OSAHS)患者血清瘦素和甲状腺素水平的相关性以及两者在OSAHS病情进展中的作用。方法选择OSAHS患者(OSAHS组)和健康者(对照组)各40例,均为男性,分别行多导睡眠图检查,并测定空腹瘦素和甲状腺素水平。结果(1)OSAHS患者的血清瘦素、FT3以及颈围分别高于对照组(P<0.05)。且随着病情的加重,血清瘦素及FT3水平逐渐增加。(2)OSAHS患者血清瘦素水平与呼吸暂停低通气指数(AHI)、呼吸障碍事件最长时间(LAHD)、微觉醒指数(MI)均呈正相关(P<0.05),与最低血氧饱和度(MOS)呈负相关(P<0.05)。(3)OSAHS患者血清瘦素水平与FT3呈正相关(P<0.05)。(4)OSAHS患者FT3与AHI及MI均呈正相关(P<0.05),FT4仅与MI呈正相关(P<0.05)。(5)OSAHS患者FT3与颈围呈正相关(P<0.05),TSH与颈围呈负相关(P<0.05),TSH与脂肪百分比呈正相关(P<0.05)。结论(1)OSAHS患者的血清瘦素水平及颈围明显升高,且与病情严重程度相关。(2)OSAHS患者FT3浓度明显增高,且与颈围存在相关性,同时发现甲状腺素与OSAHS病情严重程度存在相关性。(3)OSAHS患者瘦素水平与FT3之间存在明显的相关性。Objective To investigate the correlation between serum leptin levels and thyroxine in patients with obstructive sleep apnea-hypopnea syndrome （OSAHS） and to explore the role of serum leptin and thyroxine in the development of OSAHS. Methods The polysom nography was examined during sleep and the levels of fasting plasma serum leptin and thyroxine were determined in 40 male patients with OSAHS and 40 male healthy subjects （control group）.Results （1）As compared with those in control group,the levels of serum leptin,FT3 and neck circumference were significantly higher than those in obese and non-obese OSAHS patients （P0.05）.Moreover,serum leptin and FT3 levels became still higher when the disease condition of OSAHS patients became more severe.（2）In OSHAS group,serum leptin levels were positively correlated with AHI,LAHD,MI（P0.05）.However,serum leptin levels were negatively correlated with MOS（P0.05）.（3）In OSHAS group,serum leptin levels were positively correlated with FT3（P0.05）.（4）There was a positive correlation between FT3 and AHI,MI,however,FT4 was positively correlated with MI only（P0.05）.（5）In OSAHS group,FT3 levels were positively correlated with NC（P0.05）,TSH levels were negatively correlated with NC（P0.05）and were positively correlated with the percentage of fat（P0.05）.Conclusion （1）The serum leptin levels and NC are significantly higher in patients with OSAHS than those in healthy subjects,and which are correlated with the severity of OSAHS.（2）The FT3 levels are significantly increased in OSAHS patients,which are positively correlated with NC.The thyroxine levels are correlated with the severity of OSAHS.（3）There is a significant correlation between serum leptin and FT3 in OSAHS patients.
