重庆市农村三级医疗卫生机构医生对卫生适宜技术的需求研究

摘　　要：目的调查重庆市农村三级医疗卫生机构的医生对卫生适宜技术的需求情况及原因,为农村卫生适宜技术的筛选提供依据。方法采用现场调查研究,以目标条件抽样和分层随机抽样相结合的方法,对5个项目县的医生进行问卷调查。结果农村三级医疗卫生机构医生对适宜技术的知晓率较低。对于适宜技术的特点,县、村卫生机构侧重疗效,乡镇卫生机构侧重经济性。对技术的需求方面,县、乡卫生机构主要集中于西医技术方面,需求的主要原因为患者多、目前缺少其他有效技术;村卫生机构主要集中于农村常见病的中医技术方面,需求的原因为目前缺少其他有效技术,其不愿意参加适宜技术培训的主要原因是无法承担相应费用。结论县、乡、村卫生机构医生的适宜技术需求不同,卫生政策制定者需综合考虑各方面因素,筛选出适合当地县、乡、村各级卫生机构的适宜技术。Objective To investigate the situation of demands of rural doctors for the appropriate technology and their reasons, in order to provide an evidence about how to screen the appropriate technology for rural areas. Methods The field questionnaire survey was conducted among the doctors from five project counties of Chongqing Metropolitan by combined method of target condition sampling and stratified random sampling. Results The doctors in rural health organizations had a lower awareness of the appropriate technology. In the county and village health organizations the curative effect was emphasized, while in the township the economical feature was focused on. In the county and township organizations the technique was concentrated in Western medicine, the main reasons for demand were that there were more patients and the other techniques were lack. While in the village organizations the technique was on the rural common diseases, the main reasons for demand were that the other techniques were lack so far, and that they could not afford the fees for the training was the main reason they did not want to participate in the training. Conclusion The doctors from county, town and village have different demand for appropriate technology. So the health policy makers need to comprehensively consider various factors to draw up the appropriate technologies suitable for county, township and village health services respectively.
