社区护理干预对老年高血压疗效影响的Meta分析

摘　　要：目的采用Meta分析的方法探讨社区护理干预对老年高血压疗效的影响。方法检索中国生物医学文献数据库、维普中文科技期刊全文数据库和中国期刊全文数据库,纳入老年高血压社区护理干预的随机对照实验,由2人独立进行数据提取和文献评价。采用RevMan5.0软件进行分析。结果共纳入6篇文献,Meta分析显示社区护理干预组与对照组在高血压控制率[RR=1.55,95%CI(1.37,1.77),P<0.00001]、遵医嘱率[RR=1.68,95%CI(1.09,2.59),P=0.02]、不良生活方式改变率[RR=1.61,95%CI(1.24,2.09),P=0.0004]、疾病知识掌握[RR=1.53,95%CI(1.05,2.22),P=0.03]、收缩压[WMD=-21.10,95%CI(-23.90,-18.31),P<0.00001]和舒张压[WMD=-10.63,95%CI(-13.17,-8.09),P<0.00001]的改变方面差异均具有统计学意义。敏感性分析显示本评价结果较稳定。结论Meta分析显示社区护理干预对老年高血压的疗效优于传统护理方法。由于纳入分析的研究质量较低,需要进一步高质量的研究来揭示社区护理干预对老年高血压疗效的影响。Objective To evaluate the curative effect of community nursing intervention for hypertension in elders by using the Meta - analysis. Methods The databases of CBM, VIP and CNKI were searched. All randomized controlled trials （RCT） about community nursing intervention for elder hypertension were included. Data collection and literature review were performed by two reviewers independently. The RevMan 5. 0 software was take for analysis. Results Six literatures were included. The Meta - analysis showed there were significant differences between the community nursing intervention group and the control group in hypertension control rate [ RR = 1.55, 95% CI （ 1.37, 1.77 ）, P 〈 0. 00001 ], changing rate of unhealthy life styles [ RR = 1.68, 95% CI （ 1. 09, 2. 59 ）, P = 0.02 ], rate of mastering knowledge of hypertension [ RR = 1.61, 95% CI （ 1.24, 2.09）, P = 0. 0004 ], rate of obeying doctor＇s order [ RR = 1.53, 95% CI （ 1.05, 2. 22）, P = 0. 031, alteration of systolic blood pressure [ WMD = - 21.10, 95% CI （ - 23.90, - 18.31 ）, P 〈 0. 00001 ], and alteration of diastolic blood pressure [WMD= -10.63, 95%CI （ -13.17, -8.09）, P〈0.00001]. Conclusion Meta-analysis showsthat community nursing intervention is better than traditional nursing for eider hypertension. However, most trials included in the review are of low quality, so large - scale randomized controlled trials of higher quality are needed to confirm the result.
