超选择性肾动脉栓塞治疗损伤性肾出血15例临床分析

摘　　要：目的分析超选择性肾动脉栓塞治疗损伤性肾出血的临床疗效。方法对损伤性肾出血患者(经皮肾镜取石11例、肾穿刺活检1例、肾挫伤3例)行超选择性肾动脉栓塞治疗。使用钢圈(部分混合明胶海绵条)超选择性肾动脉栓塞15例,其中1例行二次造影和二次栓塞治疗,1例行二次造影和一次栓塞治疗。结果15例经血管造影诊断:肾脏假性动脉瘤15例,栓塞后即刻止血14例,1例第二次栓塞后即刻止血。有效率为93.3%(14/15)。全部病例无并发症发生。结论超选择性肾动脉栓塞术能最大限度保留肾脏功能,是一种安全、有效,微创的止血方法。Objective To analyze the clinical value of super selective renal arterial embolization in treatment of traumatic renal hemorrhage.Methods Fifteen patients with minimally invasive percutaneous nephrolithotomy in eleven cases,renal needle biopsy in one case and contusion in three cases underwent embolized treatment of renal arteriography.Then renal artery embolization were performed by coil and partly mixed gelatin sponge strips in fifteen cases.One case underwent two angiographic exams and two embolization in,and two angiographic exams and one embolization in another case.Results Fifteen cases underwent angiographic exam and diagnosis;there were fifteen cases with renal pseudoaneurysm,the hemoptysis was immediately stopped in fourteen cases after operation and one case in the second super selective renal arterial embolization.The effective rate was 93.3%（14/15）.There weren＇t any severe complications in the entire patient.Conclusion Super selective renal arterial embolization therapy can keep most kidneys functioning safely,effectively and using a less invasive method in treatment of renal hemorrhage.
