摘　　要：出院患者延续性护理是整体护理的一部分及住院护理的延伸,具有良好的社会效益及经济效益,在国外及我国的香港已经得到验证。文章简要概述了国内出院患者对延续性护理的需求,国内开展延续性护理的方式、内容及存在的问题,并提出了相应的对策。Continuing care of discharged patients,as a part of holistic nursing care and the extend care for the inpatients,has good economic and social benefits,which is verified in abroad and Hong Kong of China.This article summarized the demands of discharged patients for continuing nursing care,the established nursing way and content and the existed problems in China,and put forward some corresponding countermeasures.
