社区综合干预联合复方异丙托溴铵气雾剂治疗1年对COPD患者急性加重和生活质量的影响

摘　　要：目的:评估社区综合干预加复方异丙托溴铵气雾剂治疗对慢性阻塞性肺疾病(COPD)急性加重及生活质量的影响。方法:选取广州的2个社区的73例COPD患者,随机分为干预组和对照组,干预组在进行社区综合干预的基础上加复方异丙托溴铵气雾剂治疗1年,对照组只进行日常卫生服务。评估其急性加重的情况和生活质量(采用圣乔治生活质量问卷(SGRQ)进行评分)。结果:59例(80.8%)符合方案。与对照组相比,干预组的住院人数、门诊人数、住院天数、门诊次数、中度发作次数、中度发作天数、总加重发作次数减少,差异有统计学意义。干预组的SGRQ症状分数较对照组显著改善(-16.7±17.1vs-2.7±17.3),差异有统计学意义(F=8.664,P=0.005)。结论:社区综合干预加复方异丙托溴铵气雾剂治疗能够减少COPD急性加重的频率和严重程度,改善患者的生活质量。Objective： To evaluate the effect of comprehensive intervention and treatment with ipratropium bromide aerosol compound for 1 year on acute exacerbation and the quality of life of chronic obstructive pulmonary disease （COPD）. Methods： 73 patients with COPD in communities in Guangzhou were randomly enrolled into two groups, one was an intervention group, where a comprehensive intervention and treatment with ipratropium bromide aerosol compound for 1 year were performed, as well as routine health care; the other was a control group, where routine health care was conducted. Acute exacerbation of COPD was assessed, and quality of life was assessed as well using St. George＇s Quality of Life Question- naire （SGRQ）. Results： Compared with control group, intervention group had statistically fewer persons and days of hospital admissions, fewer persons and numbers of clinic visits, less frequency and days with moderate attack, less numbers of acute exacerbations in total. In the intervention group, patients had statistically significant decrease symptomatic scores of SGRQ in comparison to the control group （-16.7±17.1 vs -2.7±17.3, F=8.664,P=0.005）. Conclusion： Comprehensive intervention and treatment with ipratropium bromide aerosol compound can reduce the frequency and severity of acute exacerba- tions of COPD and improve patients＇ quality of life
