摘 要:股骨干中下及下1/3部位的骨折使用传统的Kuntscher针无法控制骨折端旋转,而内锁髓针(Interlockingnail)又由于所需设备昂贵及手术困难等原因,使推广受到限制。为此我们设计了由内外针组合而成的组合式防旋髓内针。生物力学试验表明,该针有足够的强度承受骨折部位的弯矩和扭矩,并且外加扭矩在侧刃上均匀分布,避免了内锁针的应力集中现象。临床治疗46例股骨干中下及下部骨折,平均随访34.5个月,优良率达97.8%。AIN was designed by the authors and it was composed of two parts, the inner nail and the outer nail. The biomechanical experiments showed: (1) The stiffiness of CAIN was enough to resist the bending and twisting moments at the site of the fracture and (2) The twisting moment was transmitted evenly to both side edges, thus avoiding the concentration of the stresses. From Jan 1992 to May 1993, 45 cases (46 femurs) with fractures of distal half of femoral shaft were treated by CAIN and were followed up for an average of 34.5 months. The excellent rate was 97.8%.
