血浆BNP对小儿重症肺炎合并心衰的诊断价值

摘　　要：目的:评估B型钠尿肽(B-type natriuretic peptide,BNP)对小儿重症肺炎合并心力衰竭的诊断价值。方法:利用电化学发光法测定70例肺炎患儿血浆BNP水平,与对照组比较分析,并绘制出血浆BNP的工作特征(ROC)曲线,找出诊断重症肺炎合并心力衰竭的最佳临界点。结果:重症肺炎合并心力衰竭组血浆BNP水平显著高于单纯肺炎组(P<0.01)及正常组(P<0.01);BNP作为诊断重症肺炎合并心力衰竭的临界点为105pg/mL,此时灵敏度为94.7%,特异度为85.5%,ROC曲线下的面积是0.832。结论:血清BNP是一个判断小儿重症肺炎是否合并心力衰竭较为可靠的诊断指标。Objective：To evaluate the diagnostic value of plasma B-type natriuretic peptide（BNP）for heart failure complicated by severe pneumonia in children.Method：The level of plasma BNP was measured by electrochemiluminescence immunoassay in 70 children with pneumonia and 20 healthy children as control.The receiver operating characteristics（ROC）curve of plasma BNP was created and the cut-off point of plasma BNP for the diagnosis of heart failure complicated by severe pneumonia in children was found out.Result：The levels of plasma BNP in the heart failure group was siganificantly higher than those in the pneumonia group and control group（P〈0.01）;When the cut-off point of plasma BNP for the diagnosis of heart failure complicated by severe pneumonia in children was 105 pg/mL,the sensitivity was 94.7%,the specificity was 85.5%,And the area under receiver operation characteristics curve was 0.832.Conclusion：The plasma BNP may be a reliable index in the diagnosis of heart failure complicated by severe pneumonia in children.
