彩色多普勒超声诊断精索静脉曲张程度的临床研究

摘　　要：目的探讨彩色多普勒超声诊断精索静脉曲张的临床价值。方法 97例精索静脉曲张(varicocele,VC)患者行彩色多普勒超声检测,检测平静呼吸时精索静脉最大内径(Dm),Valsalva试验时最大内径(Dv)、最大反流速度(Vm)及反流持续时间(Tr)。根据精索静脉内径将其分为4组:亚临床型精索静脉曲张(subclinical varicocele,SVC)组、VCⅠ级组、VCⅡ级组、VCⅢ级组,并随机选取35例健康志愿者为正常对照,采用单因素方差分析评价各组间差异。结果彩色多普勒超声显示VC精索静脉内径增宽及反流血流;与对照组相比,各VC组Dm、Dv、Vm、Tr明显增高(P<0.05),随着VC的严重程度增加,Dm、DV、Vm及Tr逐渐增大。结论彩色多普勒超声对诊断精索静脉曲张程度有重要参考价值。Objective To evaluate the color Doppler ultrasound in the diagnosis of varicocele degree. Methods Thirty-five healthy volunteers and 97 men with varicocele were detected by color doppler ultrasound. The longest diameter（Dm） of spermatic vein in calm breathing, the diameter（Dv）,the counter-flow velocity（Vm） and the duration of reflux（Tr） in Valsalva test were measured. According to the diameter of varicocele,97 men were divided into 4 groups： subclinical varicocele group（SVC）,clinical varicocele with grade Ⅰ as VC Ⅰ ,grade Ⅱ as VC Ⅱ and grade Ⅲ as VC Ⅲ. The differences among groups were analyzed by ANOVA test. Results Color Doppler ultrasound showed widened VC spermatic vein diameter and venous reflux.Compared with the control,Dm,Dv,Vm and Tr of VC were all increased in patients （P〈0.05）. Morover, as the severity of VC increased, Dm, Dv, Vm and Tr were correspondingly increased（P〈0.05）. Conclusion Color Doppler ultrasound is valuable for the diagnosis in patients with VC.
