摘 要:目的评估血铅对儿童红细胞免疫黏附功能的影响,探讨铅负荷对红细胞免疫黏附的影响程度以及血铅临界值与红细胞免疫黏附的关系。方法120例儿童根据血铅值分为3组:≥0.48μmol/L组(26例),0.24μmol/L~组(40例),<0.24μmol/L组(54例)。采用石墨炉原子吸收光谱法测定血铅水平。细胞免疫黏附测定采用新鲜血免疫黏附酵母菌花环实验进行红细胞C3b(RBC-C3b)受体花环率及红细胞免疫复合物(RBC-IC)花环率检测并进行比较。结果高铅组与低铅组比较细胞免疫黏附率(RICR)差异有统计学意义。①≥0.48μmol/L组儿童RBC-C3b受体花环率表达明显低于<0.24μmol/L组(t=3.78,2.67,P<0.01),而RBC-IC花环率影响不大。②血铅值≥0.48μmol/L时血铅水平与RBC-C3b受体花环率表达呈负相关(r=-0.803,-0.597,P<0.05)。结论铅对小儿细胞免疫黏附功能有不良影响,两者存在相关关系。Objective To evaluate the effects of blood lead burden on immune adherence of erythrocytes.Methods Blood lead level was measured with graphite furnace atomic absorption spectroscopy.The erythrocyte immune adherencew was reflected by percentage of rosette formation with RBC-C3b and RBC-immune complex(RBCIC) in fresh blood to yeast immune adherence test.Formation of rosette was defined as adherence of 2 or more yeasts to erythrocytes.Each 100 of RBC-C3b and RBC-IC were counted for percentage of rosette formation.The findings were put to a database and subjected for statistical analysis.Results There was a significant difference in immune adherence between investigated children with high and low lead burden.Children with blood lead level ≥0.48 μmol/L showed smaller percentage of RBC-C3b rosette formation as compared to those with blood lead level 0.24 μmol/L(t= 3.78,2.67,P〈0.01).There was a negative correlation between blood lead level and percentage of RBC-C3b rosette formation(r=-0.803,-0.597,P〈0.05).Similar results were not found for RBC-IC rosette formation.Conclusion High blood lead level may be harmful to cell immune adherence in children.
