我国农村合作医疗制度60年的变革及启示

摘　　要：合作医疗制度是具有中国特色的农村医疗保障制度,在保障农民获得基本卫生服务、缓解农民"因病致贫"和"因病返贫"方面发挥了重要的作用。建国60年来,我国农村合作医疗制度的发展经历了传统农村合作医疗制度和新型农村合作医疗制度两个发展阶段。回顾我国农村合作医疗制度60年的发展变革历程,总结和分析其发展中的经验教训,可以得到如下启示:完善的制度设计是确保新型农村合作医疗制度可持续发展的基础性保障;主体功能与作用的充分发挥是确保新型农村合作医疗制度可持续发展的关键;适宜的制度环境是确保新型农村合作医疗制度可持续发展的必要条件。A rural oriented medical system with Chinese characteristics, the cooperative medical system has played an important role in securing farmers＇ access to basic health services and preventing the phenomena of ＂poverty caused by illness＂ and ＂return to poverty due to illness＂. The development of the rural cooperative medical system has experienced two stages the traditional rural cooperative medical system and the new rural cooperative medical system in the 60 years since the foundation of the People＇s Republic of China in 1949. A review of the history of the rural cooperative medical system points to the following implications. First, fine-tuned system design is essential to the sustainable development of the new system. Second, allowing the full play of functions and responsibilities of its main parties is key to its sustainable development. Finally, favorable institutional environment serves as the precondition of its sustainable development.
