食管癌术后胸腔胃对呼吸功能的影响

摘　　要：目的:探讨食管癌手术后胸腔胃对呼吸功能的影响。方法:选取食管癌病人220例,行食管癌切除食管胃弓上吻合术110例,弓下吻合术110例。在手术前3d和手术后1、2、3、5、7、10、14d进行肺功能和血气分析测定,动态观察呼吸功能的变化规律。结果:弓上吻合术较弓下吻合术动脉氧分压、肺功能各项指标低。动脉氧分压于术后第二天降至最低,以后呈逐渐升高趋势,至术后14d恢复到术前的90%。肺功能各项指标于术后第二天降至最低,以后呈逐渐升高趋势,至术后14d恢复到术前的60%。结论:食管癌术后胸腔胃对呼吸功能有重大影响,弓上吻合术较弓下吻合术呼吸功能减退更加明显。Objective：To study the changing laws of the pulmonary function after the operation for esophageal and cardia cancinoma.Mathods：220 patients with esophageal and cardia cancinoma underwent esophagectomy and cardiectomy.The site of esophagogastroanastomosis was above aortic arch in 110 cases and under aortic arch in 110 cases.Patients were monitored from the 3rd preoperative day to the 14th postoperative day for pulmonary function and blood gas analysis.The dates were arranged in order and counted.Results：All index of the pulmonary function were the lowest on the 2nd postoperation day.Then they began to ascend gradually and were up to 60% of preopration on the 14rd postoperative day.PaO2 was the lowest on the 2nd postoperation day.Then it began to ascend gradually and was up to 90% of preoperation on the 14rd postoperative day.The pulmonary function and blood gas analysis were reduced for esophagastric anastomosis above aortic arch.Conclusion：The pulmonary function after esophagectomy and cardiectomy for esophageal and cardia cancinoma lowers markedly,which causes hypoxemia.It should be corrected.
