精浆弹性蛋白酶水平、白细胞定量与男性不育的关系

摘　　要：目的:探讨男性不育患者精浆弹性蛋白酶水平、白细胞定量与精液质量的关系。方法:对137例男性不育患者采用酶联免疫吸附试验(ELISA)进行精浆弹性蛋白酶水平检测,采用过氧化物酶染色法进行精液白细胞定量测定,采用清华同方CASAS-QH-Ⅲ计算机辅助精液分析仪进行精液质量分析。结果:精浆弹性蛋白酶水平异常组与对照组精液pH、精子密度、a级精子百分率、精子活力比较,差异有统计学意义(P<0.05);精浆弹性蛋白酶水平异常的隐性感染组与确证感染组精液pH、b级精子百分率、精子活力比较,差异有统计学意义(P<0.05);精浆弹性蛋白酶水平与白细胞定量呈正相关(rs=0.561,P<0.01);正常组不育患者精液白细胞含量(<1×106/ml)与异常组(≥1×106/ml)不育患者精液pH、a级精子百分率、精子活力比较,差异有统计学意义(P<0.05)。结论:精浆弹性蛋白酶及白细胞含量可能与精液质量及不育关系密切。Objective：To explore the relationship between elastase levels,leukocyte numbers and seminal plasma and semen quality of patients with male infertility.Methods：Enzyme-linked immunosorbent assay（ELISA）was used to detect 137 cases of patients with male infertility in seminal plasma elastase levels,seminal plasma interleukin was quantitatively determined by peroxidase staining,semen quality was analyzed by using CASAS-QH-Ⅲ computer-aided semen analyzer.Results：Elastase level abnormal group showed significant difference in semen pH,sperm density,the percentage of a level sperm and sperm motility compared with those of the normal group（P〈0.05）;inapparent infection elastase level group showed significant difference in semen group pH,the percentage of b level sperm and sperm motility compared with those of the confirmed infected group（P〈0.05）;the levels of seminal plasma and leukocyte elastase quantitative were positively correlated（rs=0.561,P〈0.01）;infertile patients with normal leukocyte content group（1×10^6/ml）showed significant difference in semen pH,the percentage of a level sperm and sperm motility compared with those of the abnormal group（≥1×10^6/ml）（P〈0.05）.Conclusion：Elastase levels and leukocyte numbers in seminal plasma enzyme may be closely related to semen quality and infertility.
