摘 要:目的探讨子痫前期中胰岛素样生长因子Ⅰ、Ⅱ(IGF-Ⅰ、Ⅱ)水平与新生儿出生体重的相关性。方法选择正常晚期孕妇(对照组)32例,轻度子痫前期孕妇20例、重度子痫前期孕妇20例(研究组),分别于分娩前抽取孕妇静脉血,分娩时留取新生儿脐静脉血和羊水,同时记录新生儿出生体重,应用酶联免疫吸附试验(ELISA),检测血清及羊水中IGF-Ⅰ、Ⅱ水平,并对检测结果进行分析。结果 (1)子痫前期孕妇血及羊水中IGF-Ⅰ、Ⅱ水平与新生儿体重呈正相关。(2)新生儿脐血中IGF-Ⅰ水平与新生儿体重呈正相关,IGF-Ⅱ水平与新生儿体重呈负相关。(3)正常对照组32例均为正常出生体重新生儿,子痫前期孕妇中胎儿生长受限(FGR)总发病率为27.5%,其中轻、重度子痫前期孕妇,FGR发病率分别为5%和50%(P<0.01),差异有统计学意义。结论子痫前期状态下IGF-Ⅰ、Ⅱ水平变化与FGR密切相关,重度子痫前期孕妇中FGR发病率较高。Objective : To explore the relevance between insulin - like growth factor - Ⅰ ,Ⅱ(IGF - Ⅰ ,Ⅱ) levels and neonatal birth weigh in the pre - eclampsia. Methods : Selected normal late pregnant women ( control group) 32 cases, mild pre - eclampsia pregnant women 20 cases, severe pre - eclampsia pregnant women 20 cases ( study group), extracted the venous blood of pregnant women before childbirth, collected newborn umbilical venous blood and amniotic fluid, and recorded the birth weight, applied enzymelinked immunosorbent assay (ELISA) to detect IGF - Ⅰ , Ⅱ levels in the serum and amniotic fluid, and analyzed test results. Results : ( 1 ) In pre - eclampsia, the IGF - Ⅰ , Ⅱ levels in the maternal serum and amniotic fluid were positively correlated with birth weight; (2) In newborn cord blood, IGF- Ⅰ level was positively correlated with neonatal body weight, while IGF - Ⅱ level was negatively correlated with birth weight; (3) In the normal control group, 32 cases were normal birth weight infants, but the fetal growth restriction (FGR) total incidence rate was 27.5% in the pre -eclampsia pregnant women, of which mild and severe pre -eclampsia pregnant women, the FGR incidence rates were 5% and 50% respectively ( P 〈0. 01 ), the difference was statistically significant. Conclusion: IGF - Ⅰ , Ⅱ levels change was closely related with FGR in the pre -eclampsia state, there was a higher incidence of FGR in severe pre - eclampsia pregnant women.
