PICC导管对病人静脉内径和血流速度的影响

摘　　要：[目的]观察经外周置入中心静脉导管(PICC)置管前后外周静脉血管内径和血流速度的变化情况。[方法]在彩色超声下观察30例经贵要静脉置入4Fr-PICC导管病人的被穿刺血管,记录穿刺前后贵要静脉、腋静脉、锁骨下静脉的血管内径及穿刺前后贵要静脉固定点血液流速。[结果]穿刺前贵要静脉内径0.247cm±0.068cm,腋静脉内径0.363cm±0.109cm,锁骨下静脉内径0.616cm±0.201cm;穿刺后贵要静脉内径为0.285cm±0.088cm,腋静脉内径0.431cm±0.126cm,锁骨下静脉内径0.696cm±0.245cm;穿刺前后改变具有统计学意义(P<0.05),且血管内径改变值呈正态分布,但与血管穿刺前内径无相关性;贵要静脉段血流速度无明显改变(P>0.05)。[结论]PICC置管后相应静脉血管内径有所扩张,但血流速度无明显改变。Objective：To observe the changes of internal diameter of peripheral vein and blood flow velocity before and after peripherally inserted central catheter.Methods：A total of 30 patients accepting peripherally inserted central catheter of 4Fr via basilic vein were selected and observed under color ultrasound.Inner diameter of basilic vein,axillary vein and subclavia vein and blood flow velocity of fixed point of basilic vein of all cases before and after puncturing were recorded by color ultrasound.Results：The inner diameters of basilic vein,axillary vein and subclavia vein were 0.247 cm±0.068 cm,0.363 cm±0.109 cm and 0.616 cm±0.201 cm respectively before puncturing.However,the inner diameters of basilic vein,axillary vein and subclavia vein were 0.285 cm±0.088 cm,0.431 cm±0.126 cm and 0.696 cm±0.245 cm after puncturing respectively.The changes were statistically significant between before and after puncturing（P0.05） and presented a normal distribution.However,it has no correlation with the inner diameter before inserting the catheter.The blood flow velocity in basilic vein had no significant changes（P0.05）.Conclusion：The inner diameter of the vein after peripherally inserted central catheter expanded a little.However,the blood flow velocity in the vein has no significant change.
