高频彩色多普勒超声显像在诊断睾丸损伤类型中的应用

摘　　要：目的探讨超声图像上睾丸损伤的影像学表现类型,从而对睾丸损伤程度做出准确判断,为临床治疗方案的制定提供有效的依据。方法收集我院外伤性睾丸疾病患者31例,应用高频彩色超声多普勒进行诊断,根据睾丸白膜回声是否连续、内部回声有无改变、彩色血流的分布以及睾丸鞘膜腔内有无积液加以分类,并与临床诊断结果进行比较。结果31例受损伤的睾丸中,破裂型9例(29%),睾丸血肿型8例(26%),睾丸挫伤型11例(36%)。此外,尚发现3例特殊类型的睾丸损伤,包括高回声型1例(3%),类肿瘤型1例(3%)和高血流灌注型1例(3%)。影像学诊断与临床诊断符合率达100%。结论不同程度的睾丸损伤会产生不同类型的声像图表现;在同一类型的不同阶段图像表现也不一样;彩色超声多普勒检查是睾丸外伤的首选方法,而且是一种诊断符合率非常高的检查方法。Objective To explore the sonography types of testicular trauma,thereby make a exact judgement for the testicular trauma,and provide effective foundation for clinical therapeutic regimen.Methods Color doppler flow image（CDFI）was performed to examine 31 damaged testicles.The echoes of the albuginea,parenchyma,and flow in testis were analysed.If testicular tunica vaginalis was filled with color fluid,the echoes should be classified.Results Among the 31 injuried testis,there were some common types of testicular trauma,but also some special types,such as high echo type,high hydrops type and parallel tumor type.Conclusion The sonography appearance of the testicular trauma was very different from each other according to the degree of injury,and in the different periods the image will be different.COFI may be the first-selected examination method with high diagnose accordance rate.
