潜在的血源传播患者梅毒血清学检测方法的选择

摘　　要：目的为潜在血源传播患者选择高灵敏度的梅毒血清学检测方法。方法采用回顾性研究,探讨潜在血源传播患者和健康体检者甲苯胺红不加热血清反应素试验(TRUST)、苍白密螺旋体胶凝集试验(TPPA),以及两者联合检测阳性率及差异。结果潜在血源传播患者组与健康体检者组的TPPA阳性率分别为5.01%和6.42%,均显著高于TRUST的阳性率,分别为2.59%和2.76%;两组人群采用同一种方法检测阳性率差异无统计学意义;在联合检测中,潜在血源传播患者组与健康体检者组出现TRUST阴性、TPPA阳性的比例分别为2.95%和2.46%。结论梅毒感染者存在于普通人群中,对于潜在血源传播患者梅毒血清学检测必须采用高灵敏度的特异性抗体检测试验,切忌单纯使用反应素检测试验筛查,避免漏检。OBJECTIVE To select a sensitive and accurate method for syphilis screening laboratory tests in patients with latent blood-transmitted risk.METHODS A retrospective analysis was adopted to study the positive rate of toluidine red unheated serum test（TRUST）and Treponema pallidum particle agglutination（TPPA）between patients with latent blood-transmitted risk and healthy volunteers during from Jun 2006 to Aug 2009.The statistic analysis method was chi-square test.RESULTS The positive rate of latent blood-transmitted risk group and healthy volunteers group the TPPA were of 5.01% and 6.42%,which were significantly higher than TRUST positive rate of 2.59% and 2.76%.There was no significant difference in positive rates in two groups.In the combination detection with TRUST and TPPA,the TRUST negative but TPPA-positive rates were 2.46% and 2.95% in the patients with latent blood-transmitted risk group and the healthy volunteers group.CONCLUSIONS Syphilis infections exist in the general population.To avoid false negatives and medical dispute we must select TPPA with high sensitivity and specificity as syphilis detection in patients with latent blood-transmitted risk,but not routine screening laboratory test TRUST.
