应用锁定钢板治疗肱骨近端骨折的临床研究

摘　　要：目的探讨应用肱骨近端锁定钢板(LPHP)治疗肱骨近端骨折的临床效果。方法选择我院自2006年3月至2008年12月收治并得到随访的23例肱骨近端骨折患者,均为Neer两部分以上骨折,均行切开复位LPHP内固定,均为新鲜骨折。最终随访应用美国肩肘外科医师协会(ASES)评分并观察肩关节活动范围评估治疗结果。结果 23例患者平均随访时间15.2个月(12～30个月),患者平均59.6岁(45～78岁),ASES评分平均84.6(70～95);肩关节活动范围平均:前屈上举146.6°(95°～170°),外旋38.2°(10°～70°),内旋达到T10(T5～L4)水平;所有随访患者无感染,无内固定失效及骨折不愈合。结论应用肱骨近端锁定钢板治疗Neer两部分以上骨折可以取得较满意的临床效果。Objective To evaluate the clinical outcomes of displaced proximal humeral fractures treated by locking proximal humeral plate.Results From March 2006 to December 2008,23 patients were treated by open reduction and internal fixation with locking proximal humeral plate （LPHP）.They were all fresh fracures.The range of motion and ASES （American Shoulder and Elbow Surgeonss Form） score were recorded at the latest follow up.The results were evaluated according to the indices listed above.Results The average time of follow-up is 15.2 month （12-30 month）.The average age of patients is 59.6 years （45～78 years old）.The mean ASES was 84.6 （70～95）.The average forward flexion was 146.6°（95°～170°）,average external rotation was 38.2°（10°～70°）and average internal rotation was T10（T5～L4）.There was no infection,fixation failure or nonunion in all patients.Conclusion Satisfactory results can be expected for the displaced proximal humeral fractures treated with locking proximal humeral plates.
