摘  要:目的 探讨β葡萄糖醛酸苷酶(βGD) 与母乳性黄疸的关系,以及母乳和粪便βGD 测定在母乳性黄疸诊断上的意义。 方法 采用酶学比色法对47 例母乳性黄疸及60 例正常新生儿母乳及粪便βGD活性浓度进行测定,并分析母乳及粪便βGD活性浓度测定在母乳性黄疸患儿的阳性率。 结果 黄疸组母乳及粪便βGD活性浓度(1-363 ±0 .299 U/L 及1-071 ±0 .297 U/L) 较正常组(0.746 ±0 .325 U/L及0 .496 ±0 .276 U/L) 明显增高,两组之间具有极显著差异(P< 0.001)。两组母乳βGD测定的阳性率分别为66-0 % 与8-3 % ,差异有显著性意义( P< 0 .01) 。其灵敏度、特异度、阳性预测值、阴性预测值及诊断正确率分别为66-0% 、91-7 % 、86-1% 、77-5 % 及80-4 % 。两组粪便βGD测定的阳性率分别为68-1 % 与6-7% ,差异有显著性意义( P<0 .01)。其灵敏度、特异度、阳性预测值、阴性预测值及诊断正确率分别为68-1% 、93-3% 、88-9% 、78-9% 及82-2 % 。 结论 βGD活性浓度与母乳性黄疸的发生、发展密切相关,母乳与粪?Objective To study the relationship between β glucuronidase(β GD) and breast milk jaundice and the diagnostic vaules of breast milk and fecal β GD measurement for breast milk jaundice in neonates. Methods Milk and fecal β GD activities were measured in 47 newborns with breast milk jaundice and in 60 healthy newborns by using enzymological method.The positive rates of milk and fecal β GD measurement in babies with breast milk jaundice were analysed. Results Milk and fecal β GD activities (1 363±0.299 U/L and 1 071±0.297 U/L) were significantly higher in infants with breast milk jaundice than those in healthy infants (0.746±0.325 U/L and 0.496±0.276 U/L).The positive rates of milk β GD measurement in infants with breast milk jaundice and healthy newborns were 66.0% and 8.3% respectively.The difference between them was significant( P <0.01).Its sensitivity,specificity,positive predicting value,negative predicting value and correct diagnostic rate were 66.0%,91.7%,86 1%,77 5% and 80 4% respectively.The positive rates of fecal β GD measurement in babies with breast milk jaundice and healthy newborns were 68.1% and 6.7% respectively,the difference between them was significant( P <0.01).Its sensitivity,specificity,positive predicting value,negative predicting value and correct diagnostic rate were 68.1%,93 3%,88 9%,78 9% and 82.2% respectively. Conclusion β GD activity seems to be an important factor in breast milk jaundice in neonates.Milk and fecal β GD measurement may be useful for diagnosis of breast milk jaundice.
